Evaluation of liver frailty index in inpatients with liver cirrhosis and analysis of the influencing factors
Objective The liver frailty index was used to evaluate inpatients with cirrhosis and the influencing factors,so as to provide a basis for the development of clinical screening evaluation and nursing intervention measures.Methods A total of 170 inpatients diagnosed with cirrhosis in the Fourth Affiliated Hospital of Xinjiang Medical University from December 2021 to September 2022 were collected.Demographic information,serological indicators,liver cirrhosis complications,weekly physical activity level,CTP score,MELD score,and nutritional assessment(BMI,dominant hand grip strength,walking speed,calf circumference,upper arm circumference,and nutritional score)were collected.Nutritional risk screening tool(NRS2002)was used for nutritional risk screening.LFI(dominant hand grip strength,balance test,chair standing test)was evaluated in patients with cirrhosis.The patients were divided into frailty group and non-frailty group with LFI as the evaluation standard,and multivariate Logistic regression analysis was used to screen the influencing factors of frailty.Results The incidence of frailty in patients with cirrhosis was 33.5%.There were significant differences in dominant hand grip strength,nutritional risk,weekly physical activity level,CTP score,MELD score and abdominal effusion between the frailty group and the non-frailty group(P<0.05).Univariate analysis showed:that red blood cell count,hemoglobin,total protein,total cholesterol,cholinesterase,cystatin C,low density lipoprotein,CTP score,CTP grade,MELD score.abdominal effusion,dominant hand grip strength,nutritional score,nutritional risk,weekly physical activity level,and physical activity level grouping were the influencing factors of frailty in liver cirrhosis(P<0.05).The multi-factor Logistic regression model was established with the indicators with statistical differences in the univariate analysis,the analysis show that:Nutritional risk[OR=2.361,95%CI(1.096,5.088),P=0.028]was a risk factor for frailty in patients with cirrhosis.The level of weekly physical activity[OR=0.999,95%CI(0.999,1.000),P=0.048]and low-density lipoprotein[OR=0.546,95%CI(0.331,0.900),P=0.018]were protective factors for frailty in patients with cirrhosis.Conclusion Improving the nutritional status and physical activity levels of patients with liver cirrhosis could reduce the incidence of frailty.
