Establishment and effectiveness testing of a risk assessment model for urinary tract infection in patients with ovarian cancer after cytoreductive surgery
Objective To construct a risk assessment model for urinary tract infection in patients with ovarian cancer after cytoreductive surgery and test its assessment effect.Methods We selected 112 ovarian cancer patients who underwent cytoreductive surgery in the hospital from February 2021 to February 2022 as the research subjects,used a general information scale to investigate patient information,and counted the number of patients with postoperative urinary tract infection,and screened the influencing factors of postoperative urinary tract infection through univariate analysis and multivariate Logistic regression analysis,constructed a urinary tract infection risk assessment model based on the regression analysis results,and tested its effect through application.Results Statistics showed that 24 ovarian cancer patients developed urinary tract infection after cytoreduction,with an incidence rate of 21.43%.Multivariate Logistic regression analysis showed that the age,diabetes mellitus,operation time,intraoperative blood loss,postoperative catheter indwelling time,catheter intubation times,postoperative hypoalbuminemia,and education on urinary tract infection prevention were influencing factors of urinary tract infection in patients with ovarian cancer after cytoreductive surgery(P<0.05).Among them,postoperative patients who were ≥65 years old,had diabetes,had a urinary catheter indwelling for ≥7 days,had catheterizations>1 times,had postoperative hypoalbuminemia,and lack of education on urinary tract infection prevention were more likely to develop urinary tract infection.Based on the results of logistic regression analysis,a urinary tract infection risk assessment model for ovarian cancer patients after cytoreductive surgery was constructed.The Hosmer-Lemeshow test of the model showed that the difference was not statistically significant(x2=2.848,P=0.132),and the goodness of fit of the model was good;the AUC of the model ROC curve was 0.805,the 95%CI was 0.729~0.877,the maximum Youden index was 0.649,and the corresponding sensitivity and specificity were 0.802 and 0.847 respectively;clinical application testing showed that the model prediction accuracy was 91.96%.Conclusion Patients with ovarian cancer were associated with varying degrees of risk of urinary tract infection after cytoreductive surgery,the age,diabetes mellitus,operation time,intraoperative blood loss,postoperative catheter indwelling time,catheter intubation times,postoperative hypoalbuminemia,and education on urinary tract infection prevention were influencing factors of urinary tract infection in patients with ovarian cancer after cytoreductive surgery.The constructed model can effectively identify the risk of urinary tract infection after cytoreduction for ovarian cancer,and provide a reference for clinical preventive care to enhance the pertinence of care and thereby reduce the incidence of urinary tract infection.
