Implementation effect of the medical alliance health management model based on health ecology theory in patients with chronic heart failure
Objective To explore and analyze the impact of a medical alliance health management model based on health ecology theory on self-management of patients with chronic heart failure,in order to improve their self-management level and quality of life.Methods A total of 218 patients with chronic heart failure in two community health service centers under the jurisdiction of a tertiary Grade A hospital in Nanjing from November 2020 to February 2021 were selected as the research subjects.According to the principle of matching basic data between groups,they were divided into an observation group of 115 cases and a control group of 103 cases.The observation group adopted the health management model of medical alliance based on the theory of health ecology,including peer support for chronic heart failure patients with diversified information support led by"Internet+"and professional support provided by hospitals and community health service centers through the platform of"medical alliance";the control group received chronic heart failure health management education,which was taught through a combination of health education lectures and interactive teaching methods.After an intervention period of 12 months,self-care of heart failure index scale and Minnesota living with heart failure questionnaire were used to evaluate and compare the changes in patient self-management and quality of life before and after the intervention;the patients'satisfaction was evaluated by using a satisfaction scale.Results After 12 months of health management intervention,the self-management score in the observation group was higher than that in the control group,the quality of life score was lower than that in the control group,and the differences between the groups were statistically significant(P<0.05).The patients'satisfaction with community hospitals was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The health management model of medical alliance based on health ecology theory can help improve the self-management ability of patients with chronic heart failure,improve their quality of life,and increase satisfaction.
