Risk factors analysis for complication of CRT in oncology patients with upper arm-type infusion port
Objective To explore the risk factors of catheter-related thrombosis(CRT)in patients with upper arm infusion port tumors,to provide a certain reference basis for the development of clinical nursing program. Methods 120 patients with upper arm-type infusion port tumors admitted to the hospital from February 2021 to February 2022 were selected as the study subjects,36 patients with CRT occurence were set as the case group and 84 patients without CRT Occurence as the control group. The basic data of the patients and the occurrence of catheter-related thrombosis were collected by the questionnaires, the risk factors for the occurrence of catheter-related thrombosis in patients with upper arm-type infusion port tumors were explored by means of univariate analysis and multifactorial Logistic regression analysis. Results The results of the present investigation showed that among 36 CRT patients in this study, there were 9 cases of symptomatic thrombosis and 27 cases of asymptomatic thrombosis. The results of unifactorial and multifactorial Logistic regression analyses showed that patients'age≥60 years, higher BMI,tumor stage III-IV,higher TC level, higher FIB,higher D-dimer level, and higher risk of concurrent CRT in patients with tumors in the port of the upper arm infusion(P<0.05).Conclusion CRT is a common complication in patients with upper arm-type infusion port tumors, and its occurrence will increase the cost of treatment for patients and bring additional burden for treatment, the study found that asymptomatic thrombosis was most common in patients with upper arm-type infusion port tumors after investigation. Moreover, multifactorial regression analysis in this study showed that patients age,BMI,tumor stage,TC level,FIB level, and D-dimer level were independent risk factors for the complication of CRT in patients with upper arm infusion port tumors, suggesting that clinics should pay attention to the assessment of the above factors and fully applied the basic nursing interventions, including close monitoring of patients'symptoms, anticoagulant therapy, and keeping the catheter and the surrounding area clean, etc.and reference to the corresponding management guidelines to formulate the corresponding protection program,in order to reduce the risk of CRT in patients with tumors in the upper arm infusion port,to protect the effectiveness and continuity of patient treatment.
