血清VEGF和PLGF联合对子痫前期预测价值分析

摘要
目的 探讨血清血管内皮生长因子(VEGF)及胎盘生长因子(PLGF)联合对子痫前期的预测价值.方法 选取2020年1月～2022年1月在我院产科就诊的子痫前期患者60例(子痫前期组)为研究对象,其中轻度子痫前期36例,重度子痫前期24例,另选取同期在我院就诊的30例正常孕妇为正常妊娠组.检测并比较两组血清VEGF及PLGF水平,采用受试者工作特征(ROC)曲线评估血清VEGF及PLGF联合检测对子痫前期的预测价值.结果 纳入本研究的60例子痫前期患者中,重度子痫前期组患者血清 VEGF、PLGF 水平[(21.07±2.81)ng/L、(43.52±9.47)ng/L]低于轻度子痫前期组[(30.85±2.81)ng/L、(60.33±12.58)ng/L],二者血清 VEGF、PLGF 水平均低于正常妊娠组[(36.11±5.06)ng/L、(75.12±14.31)ng/L](P＜0.05).ROC 曲线分析显示,血清VEGF联合PLGF检测对子痫前期的预测敏感度、特异度分别为90.2％、90.0％,均高于VEGF检测的76.7％、88.3％和PLGF检测的83.3％、75.0％.结论 子痫前期孕妇血清VEGF、PLGF水平均显著降低,早期联合动态监测其水平变化有助于提高子痫前期的诊断效能.
Abstract
Objective To investigate the predictive effect of serum vascular endothelial growth factor(VEGF)and placental growth factor(PLGF)on preeclampsia.Methods A total of 60 patients with preeclampsia(pre-eclampsia group)treated in the obstetrics department of our hospital from January 2020 to January 2022 were selected as research objects,including 36 patients with mild preeclampsia and 24 patients with severe preeclampsia,and 30 normal pregnant women treated in our hospital during the same period were selected as normal pregnancy group.The serum VEGF and PLGF levels were detected and compared between the two groups.The predictive value of combined detection of serum VEGF and PLGF in preeclampsia was evaluated by the receiver operating characteristic(ROC)curve.Results The serum levels of VEGF and PLGF[(21.07±2.81)ng/L,(43.52±9.47)ng/L]in the severe preeclampsia group were lower than those in the mild pregnancy group[(30.85±2.81)ng/L,(60.33±12.58)g/L].The levels of serum VEGF and PLGF in both groups were lower than those in normal pregnancy group[(36.11±5.06)ng/L,(75.12±14.31)ng/L](P＜0.05).ROC curve analysis showed that the sensitivity and specificity of serum VEGF combined with PL-GF detection for the prediction of preeclampsia were 90.2％and 90.0％,respectively,higher than that of VEGF detec-tion(76.7％and 88.3％)and PLGF detection(83.3％and 75.0％).Conclusion Serum VEGF and PLGF levels are significantly decreased in pregnant women with preeclampsia,and early combined dynamic monitoring of their levels is helpful to improve the diagnostic efficacy of preeclampsia.
