术前抑郁对单髁关节置换术后疼痛的影响

摘要
目的 探讨术前抑郁对单髁关节置换患者术后疼痛的影响.方法 筛选2021年3月～2023年3月潍坊市中医院关节骨科收治的膝关节骨性关节炎(KOA)行单髁关节置换术(UKA)患者共150例,平均年龄(63.4±5.1)岁.应用焦虑及抑郁自评量表(SDS)对患者进行评分,根据评分结果,其中抑郁患者55例为抑郁组,从非抑郁患者95例中随机选择60例为非抑郁组.分别于术后7d、14d应用视觉模拟评分法(VAS)对患者疼痛程度评估;用Pearson法分析术后7d、14d VAS评分与术前SDS评分的相关性.结果 两组患者术后7d VAS评分(4.470±1.359,3.280±1.151)均高于同组术后 14d VAS 评分(3.620±1.312,2.680±0.965)(P＜0.001);非抑郁组患者术后7d、14d的VAS评分均低于抑郁组(P＜0.001);两组患者术后7d、14d VAS评分与术前SDS评分呈正相关(P＜0.05).结论 术前抑郁可加重UKA患者术后疼痛,进而影响其生活质量和临床治疗效果.可通过控制调节患者术前抑郁程度,降低患者术后疼痛,提高满意度.
Abstract
Objective To explore the influence of preoperative depression on postoperative pain in patients undergoing uni-compartmental knee arthroplasty(UKA).Methods A total of 150 patients undergoing UKA for KOA were divided into depression group(n=55)and no-depression group(n=60)according to the Self-Rating Depression Scale(SDS)before operation.The Visual Analogus Scale(VAS)was adopted to evaluate the pain degree on the 7th day and the 14th day after the operation.Pearson correlation method was used to analyze the correlation of VAS score on the 7th day and the 14th day after operation with the preoperative SDS score.Results The VAS scores were significantly higher on the 7th day after operation(4.470±1.359,3.280±1.151)than the 14th day after operation(3.620±1.312,2.680±0.965)both in depression and no-depression groups(P＜0.001),and lower in no-depression group than those in depression group both on the 7th day and the 14th day after operation(P＜0.001).Pearsoncor rationalizes howled that VAS scores on the 7 th day and the 14th day after-operation were positively correlated with the preoperative SDS score(P＜0.05).Conclusion Preoperative depression can exacerbate postoperative pain in UKA patients,thereby affecting their quality of life and clinical treatment effectiveness.By controlling and adjusting the degree of preoperative depression in patients,postoperative pain can be reduced and satisfaction can be improved.
