2018~2023年男性HPV感染状况及基因型分布研究

摘要
目的 统计分析潍坊地区男性人群感染人乳头瘤病毒(HPV)的基因亚型分布和流行病学特征.方法 回顾性分析2018 年1 月~2023 年12 月在我院做HPV分型检测的 661 例男性的检测数据.采用实时荧光定量PCR方法测定23 种HPV基因亚型,并分析HPV各亚型占比、HPV阳性患者的年龄分布以及2018~2023年阳性率变化情况.结果 661 例样本中检测出HPV阳性 257 例,总阳性率 38.88%.在阳性样本中,单纯低危亚型占比41.25%,单纯高危亚型占比30.35%,高、低危亚型混合感染占比 28.40%.低危型以 6、11、43 亚型感染最常见,高危型以16、58、52 亚型感染最常见.各年龄组均以单一亚型感染为主,其次为双重亚型感染.<20 岁的年龄组感染率最高(70.37%),但是阳性感染者高峰年龄段主要集中在 20~40 岁.相比较于 2018 年的感染率(64.43%),2019~2023 年每年的感染率明显下降,2023 年感染率最低(27.81%),但是 2023 年总检测人数最多.结论 山东省潍坊地区男性HPV感染者以单一低危亚型感染较常见,总体感染基因型以 6、11、43、16、58、42 亚型最常见.2018~2023 年阳性率逐年下降,年度百分比变化(APC)呈现下降趋势.2020 年新型冠状病毒(COV-ID-19)流行期间,男性HPV检测者数量减少,HPV感染率下降.
Abstract
Objective To analyze the genotype distribution and epidemiological characteristics of human papilloma virus(HPV)in male patients in Wei fang city.Methods The data of 661 patients who were performed detec-tions of HPV DNA from January 2018 to December 2023 was collected and analyzed.Twenty three gene types of HPV were tested by Real-time Quantitative PCR,and the proportion of HPV gene types,age distribution and annual positive rate were analyzed.Results A total of 257 positive cases were detected from 661 samples of male,with a total positive rate of 38.88%.Among these positive samples,41.25%were infected with single low-risk HPV(LR-HPV),30.35%were infected with single high-risk HPV(HR-HPV),and 28.40%were infected with high and low risk HPV.LR-HPV were most commonly infected with genotypes 6,11 and 43,and HR-HPV were most commonly infected with subtypes 16,58 and 52.Single infection was domination in all age group,followed by double infections.The highest prevalence of in-fection was found in the age group less than 20 years(70.37%),but the peak age group of positive infections was mainly concentrated in the age group of 20~40 years.Compared to the infection rate in 2018(64.43%),the annual infection rate decreased significantly from 2019 to 2023,with the lowest rate in 2023(27.81%),but the total number of people tested in 2023 is the largest.Conclusion The LR-HPV infection was more common among males in Wei fang area,and the gene types 6,11,43,16,58 and 42 were the most common.During COVID-19 pandemic,fewer men were tested for HPV and the HPV infection rate has decreased dramatically.
