高质量发展视角下公立医院运营效率评价研究

摘要
目的 通过纵向和横向剖析公立医院运营效率现状,为全面提升医院的运营效率提供理论依据和支撑.方法 运用文献法和 Pearson 相关分析构建公立医院运营效率评价指标体系,采用三阶段 DEA-Malmquist指数模型对2018～2021 年50 家公立医院运营效率进行评价,运用绝对β收敛性检验评价全要素生产率(TFP)的收敛性.结果 剔除环境变量和随机干扰等因素的影响后,2018～2021 年总体的综合效率值分别为0.936、0.935、0.925、0.941,TFP均值为1.268,除2020 年外,均高于第一阶段效率值,样本医院TFP 呈绝对β收敛趋势.结论 三阶段DEA-Malmquist模型对样本医院运营效率评价研究有较强适用性;各医院运营效率有差异,环境变量对医院运营效率有较大影响,应优化医疗资源整体布局,提高资源使用效率,推动医院高质量发展;初始全要素生产率低的样本医院对较高医院具有"追赶效应".
Abstract
Objective To study the operational efficiency through vertical and horizontal comparison,and to analyze the current situation of public hospitals in Shandong province,we can find the theoretical basis for comprehen-sively improving the operational efficiency of public hospitals.Methods The evaluation index system of operational effi-ciency of public hospitals is established by literature research and Pearson correlation analysis.The operational efficiency of 50 sample hospitals in our province from 2018 to 2021 is evaluated by three-stage DEA-Malmquist index model,and the convergence of total factor productivity is evaluated by absolute β convergence test.Results Excluding the influence of environmental variables and random interference,the overall comprehensive efficiency values from 2018 to 2021 are 0.936,0.935,0.925 and 0.941 respectively.The average value of TFP is 1.268,which is higher than the first stage value except 2020,and the TFP of the sample hospital appears an absolute β Convergence trend.Conclusion The three-stage DEA-Malmquist model has strong applicability to the evaluation of operational efficiency of the sample hospitals.The op-erational efficiency of each hospital is different,and environmental variables have great influence on the operational effi-ciency.We should optimize the overall layout of medical resources,improve the utilization efficiency of resources,and promote high-quality development of each hospital;the sample hospitals with low initial total factor productivity have a"catch-up effect"on the higher hospitals.
