摘要
目的 利用Meta分析探究中性粒细胞/淋巴细胞比值(NLR)在社区获得性肺炎(CAP)预后评估中的临床应用价值.方法 以NLR和CAP为关键词检索Pubmed、Cochrane Library、中国知网等数据库,筛选文献,提取数据,用RevMan、MedCalc软件进行分析.结果 共纳入 25 篇文献,Meta分析结果显示,CAP 患者中,高NLR者死亡率风险为低NLR者的3.88 倍;死亡组患者NLR值明显高于存活组患者(P<0.001),平均差值为7.25;NLR预测CAP患者死亡的曲线下面积(AUC)为0.729,其中预测住院期间、30d之内和超过 30d患者死亡的AUC分别为0.736,0.738 和0.688.结论 NLR在CAP患者死亡预后评估尤其是短期死亡预后评估中有一定的临床应用价值.
Abstract
Objective To systematically investigate the clinical value of neutrophil/lymphocyte ratio(NLR)in the prognosis of community acquired pneumonia(CAP)through meta-analysis.Methods Literatures from the data-bases(such as Pubmed,Cochrane Library,and CNKI et al)were searched,screened literatures and extracted data.The keywords were neutrophil/lymphocyte and community acquired pneumonia.Then,meta-analysis was performed using RevMan and MedCalc software.Results After searching and screening,a total of 25 articles were included.The meta-a-nalysis results showed that the mortality of the higher NLR patient group was 3.88 times more than that of the lower NLR patient group.The NLR value of patients in the death group was significantly higher than that of patients in the survival group(P<0.001),with a mean difference of 7.25.The area under the curve(AUC)of NLR in assessment of CAP pa-tient total mortality was 0.729,and the AUC of in-hospital,within 30 days and over 30 days mortality were 0.736,0.738 and 0.688,separately.Conclusion NLR have a significant clinical value in the prognosis of CAP patient mortality,in especial the short-term mortality.
