血清同型半胱氨酸、Lp(a)、LDL与急性心肌梗死的关系研究

摘要
目的 分析血清同型半胱氨酸(Hcy)、脂蛋白a(Lp(a))和低密度脂蛋白(LDL-C)与急性心肌梗死(AMI)的关系.方法 选择37例急性心肌梗死患者作为观察组,另选择健康对照组20例.比较两组以及不同类型心肌梗死患者之间血清中Hcy、Lp(a)和LDL水平;使用ROC曲线分析Hcy、Lp(a)在急性心肌梗死患者中的检测值.结果 观察组血清Hcy(17.80±11.40)μmol/L、Lp(a)(23.91±20.21)mg/dl,高于对照组,差异有统计学意义(P＜0.05).两组的LDL水平没有明显差异(P＞0.05).血清Hcy、Lp(a)和LDL水平在急性ST抬高型与急性非ST抬高型心肌梗死患者之间没有统计学差异(P＞0.05).ROC曲线结果显示,血清Hcy和Lp(a)曲线下面积分别是AUC=0.714,P＜0.05和AUC=0.698,P＜0.05.结论 血清Hcy、Lp(a)均与急性心肌梗死关系密切.
Abstract
Objective To analyze the relationship between serum homocysteine(Hcy),lipoprotein a(Lp(a)),and low-density lipoprotein(LDL-C)and acute myocardial infarction(AMI).Methods Thirty-seven patients with acute myocardial infarction were selected as the observation group,and another 20 healthy individuals were selected as the control group.The serum levels of Hcy,Lp(a),and LDL were compared between the two groups and between patients with different types of myocardial infarction(acute ST-elevation and non-ST-elevation myocardial infarction).The detec-tion values of Hcy and Lp(a)in patients with acute myocardial infarction were analyzed using ROC curves.Results In the observation group,the serum Hcy level was(17.80±11.40)μmol/L,and the Lp(a)level was(23.91±20.21)mg/dl,both higher than those in the control group,and the differences were statistically significant(P＜0.05).There was no significant difference in the LDL level between the two groups(P＞0.05).There were no statistically significant differ-ences in the serum Hcy,Lp(a),and LDL levels between patients with acute ST-elevation and non-ST-elevation myocar-dial infarction(P＞0.05).The ROC curve results showed that the areas under the curves of serum Hcy and Lp(a)were AUC=0.714,P＜0.05 and AUC=0.698,P＜0.05,respectively.Conclusion Serum Hcy and Lp(a)are closely related to acute myocardial infarction.
