目的 分析芪苈强心胶囊联合伊伐布雷定治疗射血分数中间值心力衰竭(heart failure with midrange ejection fraction，Hfm-rEF)的效果以及对患者心功能、血清半乳糖凝集素3(galectin 3，Gal-3)、可溶性基质裂解素2(soluble suppression of tumorigeni-city 2，sST2)和外周血Ⅲ型前胶原氨基端肽(procollagen Ⅲ N-terminal peptide，PⅢNP)水平的影响。方法 选取接受常规抗心力衰竭治疗的98例HfmrEF患者，将口服芪苈强心胶囊联合伊伐布雷定的50例患者作为联合组、口服伊伐布雷定的48例患者作为伊伐布雷定组，比较2组患者的超声心动图指标、心肾功能指标、相关实验室指标、血浆神经内分泌激素和生活质量的改变。结果 治疗后联合组患者的左心室射血分数(left ventricular ejection fraction，LVEF)高于伊伐布雷定组，左室收缩末内径(left ven-tricular end systolic diameter，LVESD)和左房内径(left atrial diameter，LAD)均小于伊伐布雷定组(P＜0。05)。治疗后联合组N末端脑钠肽(N-terminal pro-brain natriuretic peptide，NT-proBNP)水平低于伊伐布雷定组(P＜0。05);2组血肌酐(serum creatinine，Scr)水平比较差异无统计学意义(P＞0。05)。治疗后联合组患者Gal-3、sST2和PⅢNP水平均低于伊伐布雷定组(P＜0。05)。治疗后联合组患者醛固酮(aldosterone，ALD)、去甲肾上腺素(norepinephrine，NE)和血管紧张素Ⅱ(angiotensin Ⅱ，AngⅡ)水平均低于伊伐布雷定组(P＜0。05)。治疗后联合组患者36条目简明健康状况调查问卷(short form 36 health survey questionnaire，SF-36)各维度评分均高于伊伐布雷定组(P＜0。05)。结论 芪苈强心胶囊联合伊伐布雷定可有效增加心脏供血，有助于改善心肾功能，降低Gal-3、sST2和PⅢNP水平，提高患者的生活质量。
Effect of Qili Qiangxin Capsules combined with ivabradine in the treatment of heart failure with midrange ejection fraction
Objective To analyze the effect of Qili Qiangxin Capsules combined with ivabradine on cardiac function,the levels of se-rum galectin 3(Gal-3),soluble suppression of tumorigenicity 2(sST2)and peripheral blood procollagen Ⅲ N-terminal peptide(PⅢNP)in patients with heart failure with midrange ejection fraction(HfmrEF).Methods A total of 98 patients with HfmrEF who re-ceived conventional anti-heart failure treatment in the hospital were enrolled.Among them,50 patients given oral administration of Qili Qiangxin Capsules combined with ivabradine were set as combined group,and 48 patients treated with oral administration of ivabradine were served as ivabradine group.The changes in echocardiographic indicators,cardio-renal function indicators,related laboratory indicators,plasma neuroendocrine hormones and quality of life were compared between the 2 groups.Results After treat-ment,the left ventricular ejection fraction(LVEF)of combined group were higher than those of ivabradine group,while the left ven-tricular end systolic diameter(LVESD)and the left atrial diameter(LAD)were lower than that of ivabradine group(P＜0.05).The level of N-terminal pro-brain natriuretic peptide(NT-proBNP)in combined group after treatment was lower than that in ivabradine group,while the levels of serum creatinine(Scr)showed no significant difference(P＞0.05).The levels of Gal-3,sST2 and PⅢNP of patients were lower in combined group compared with those in ivabradine group after treatment(P＜0.05).The levels of aldoste-rone(ALD),norepinephrine(NE)and angiotensin Ⅱ(Ang Ⅱ)of patients in combined group after treatment were lower than those in ivabradine group(P＜0.05).After treatment,the scores of dimensions of short form 36 health survery questionaire(36-item)short-form health survey(SF-36)in combined group were higher compared to ivabradine group after treatment(P＜0.05).Conclusion Qili Qiangxin Capsules combined with ivabradine can effectively increase heart blood supply,help improve heart-kidney function,reduce the levels of Gal-3,sST2 and PⅢNP,and improve the quality of life of patients.
