维生素A、维生素D辅助儿童重症支原体肺炎治疗的效果

目的 探讨维生素A、维生素D辅助糖皮质激素、阿奇霉素治疗儿童重症肺炎支原体肺炎(serious mycoplasma pneumoniae pneumonia,SMPP)的效果及对患儿心肌酶谱指标的影响。方法 选取134例SMPP患儿,用随机数字表法分为观察组与对照组,每组67例,对照组给予糖皮质激素、阿奇霉素治疗,观察组在对照组治疗的基础上给予维生素A、维生素D辅助治疗。比较2组患儿的临床疗效、临床症状与体征改善时间、心肌酶谱指标、炎性指标、免疫功能指标和不良反应发生情况。结果 2组临床治疗有效率比较差异无统计学意义(P>0。05);观察组退热时间、咳嗽消失时间、肺部啰音消失时间、肺部阴影消失时间、住院时间均短于对照组(P<0。05);观察组治疗后外周血维生素A、25-羟维生素D水平均高于对照组(P<0。05);观察组治疗后血清心肌酶谱指标天冬氨酸氨基转移酶(aspartate transaminase,AST)、乳酸脱氢酶(lactate dehydrogenase,LDH)、磷酸肌酸激酶(creatine kinase,CK)、磷酸肌酸激酶同工酶(creatine kinase-MB,CK-MB)水平均低于对照组(P<0。05);观察组治疗后血清炎性指标y-干扰素(interferon-y,IFN-y)水平高于对照组,白细胞介素-4(interleukin-4,IL-4)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、C反应蛋白(C-reactive protein,CRP)水平均低于对照组(P<0。05);观察组治疗后免疫球蛋白IgA、IgG、IgM水平,T淋巴细胞CD4+水平、CD4+/CD8+值高于对照组,CD8+水平低于对照组(P<0。05)。结论 维生素A、维生素D辅助糖皮质激素、阿奇霉素治疗SMPP可有效缩短患儿发热和咳喘等临床症状消退时间,加快患儿的康复进程,改善维生素A、维生素D缺乏状态,加强抗炎和机体免疫调节作用,减轻患儿的心肌损伤。
Effect of vitamin A and vitamin D in the adjuvant treatment of children with serious mycoplasma pneumoniae pneumonia
Objective To explore the curative effect of glucocorticoid and azithromycin assisted by vitamin A and vitamin D on children with serious mycoplasma pneumoniae pneumonia(SMPP)and the influences on myocardial enzymes.Methods A total of 134 children with SMPP were enrolled.According to random number table method,they were divided into observation group and control group,67 cases in each group.The control group was treated with glucocorticoid and azithromycin,while the observation group was additionally treated with vitamin A and vitamin D.The clinical curative effect,improvement time of clinical symptoms and signs,myocardial enzymes,inflammatory indexes,immune function indexes and adverse reactions were compared between the 2 groups.Results There was no significant difference in response rate of clinical treatment between the 2 groups(P>0.05).The antipyretic time,disappearance time of cough,lung rales and shadow,and hospitalization time in observation group were all shorter than those in control group(P<0.05).After treatment,the levels of peripheral blood vitamin A and 25-hydroxyvitamin D in observation group were higher than those in control group(P<0.05),and the levels of serum myocardial enzymes[aspartate transaminase(AST),lactate dehydrogenase(LDH),creatine kinase(CK),creatine kinase-MB(CK-MB)]were lower than those in control group(P<0.05).After treatment,the level of serum interferon-y(IFN-y)in observation group was higher than that in control group,while the levels of interleukin-4(IL-4),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)were lower than those in control group(P<0.05).After treatment,the levels of immunoglobulins(IgA,IgG,IgM)and T lymphocytes(CD4+,CD4+/CD8+)in observation group were higher than those in control group,while the CD8+level was lower than that in control group(P<0.05).Conclusion Glucocorticoid and azithromycin assisted by vitamin A and vitamin D can effectively shorten the disappearance time of clinical symptoms(fever,cough)in SMPP children,accelerate rehabilitation process,improve the lack of vitamins A and D,strengthen anti-inflammatory and immune regulation effects,and reduce myocardial injury.
