Effect of vitamin A and vitamin D in the adjuvant treatment of children with serious mycoplasma pneumoniae pneumonia
Objective To explore the curative effect of glucocorticoid and azithromycin assisted by vitamin A and vitamin D on children with serious mycoplasma pneumoniae pneumonia(SMPP)and the influences on myocardial enzymes.Methods A total of 134 children with SMPP were enrolled.According to random number table method,they were divided into observation group and control group,67 cases in each group.The control group was treated with glucocorticoid and azithromycin,while the observation group was additionally treated with vitamin A and vitamin D.The clinical curative effect,improvement time of clinical symptoms and signs,myocardial enzymes,inflammatory indexes,immune function indexes and adverse reactions were compared between the 2 groups.Results There was no significant difference in response rate of clinical treatment between the 2 groups(P>0.05).The antipyretic time,disappearance time of cough,lung rales and shadow,and hospitalization time in observation group were all shorter than those in control group(P<0.05).After treatment,the levels of peripheral blood vitamin A and 25-hydroxyvitamin D in observation group were higher than those in control group(P<0.05),and the levels of serum myocardial enzymes[aspartate transaminase(AST),lactate dehydrogenase(LDH),creatine kinase(CK),creatine kinase-MB(CK-MB)]were lower than those in control group(P<0.05).After treatment,the level of serum interferon-y(IFN-y)in observation group was higher than that in control group,while the levels of interleukin-4(IL-4),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)were lower than those in control group(P<0.05).After treatment,the levels of immunoglobulins(IgA,IgG,IgM)and T lymphocytes(CD4+,CD4+/CD8+)in observation group were higher than those in control group,while the CD8+level was lower than that in control group(P<0.05).Conclusion Glucocorticoid and azithromycin assisted by vitamin A and vitamin D can effectively shorten the disappearance time of clinical symptoms(fever,cough)in SMPP children,accelerate rehabilitation process,improve the lack of vitamins A and D,strengthen anti-inflammatory and immune regulation effects,and reduce myocardial injury.
