General anesthesia effect of ultrasound-guided sciatic nerve block combined with general anesthesia in elderly patients with femoral neck fractures
Objective To explore general anesthesia effect of ultrasound-guided sciatic nerve block combined with general anesthesia in elderly patients with femoral neck fractures.Methods A total of 90 elderly patients with femoral neck fractures were enrolled.According to random number table method,they were divided into observation group(n=45)and control group(n=45).The control group was given general anesthesia(fentanyl,etomidate and rocuronium bromide),while the observation group was additionally given ultrasound-guided sciatic nerve block.The extubation time,anesthesia recovery time,hospitalization time,perioperative heart rate(HR),respiration rate(RR),mean arterial pressure(MAP)and transcutaneous oxygen saturation(SPO2)in both groups were re-corded.The postoperative pain and sedation effect were evaluated by visual analogue scale(VAS)and Ramsay sedation score.The postoperative recovery quality and cognitive function were evaluated by Steward recovery scale and mini-mental state examination(MMSE).The occurrence of adverse reactions was recorded.Results The anesthesia recovery time,hospitalization time and extuba-tion time in observation group were better than those in control group(P<0.05).Before surgery(t1),the difference in HR,RR,MAP and SPO2 between the 2 groups was not statistically significant,but which were significantly lower in observation group than control group at 15 minutes during the surgery(t2),30 minutes during the surgery(t3)and after surgery(t4,P<0.05).At 2,12 and 24 hours after surgery,the VAS and Ramsay scores in observation group were significantly lower than those in control group(P<0.05).The MMSE and Steward scores in observation group were significantly higher than those in control group(P<0.05).The difference in the incidence of adverse reactions between observation group and control group was not statistically significant(8.88%vs.15.56%).Conclusion The anesthesia effect of ultrasound-guided sciatic nerve block combined with general anesthesia is good in elderly patients with femoral neck fractures,which has good analgesic and sedative effects,and is beneficial to maintain hemodynamics stability,improve recovery quality,reduce the injury to cognitive function and promote postoperative recovery.
