Effect of atosiban combined with nifedipine in patients with threatened preterm premature rupture of membranes and its influence on maternal-infant outcomes
Objective To investigate the clinical effect of atosiban combined with nifedipine in the treatment of patients with threatened preterm premature rupture of membranes and the influence on the body mass of premature infants and maternal-infant outcomes. Methods A total of 120 patients with threatened preterm premature rupture of membranes were selected and divided into control group(60 cases, nifedipine)and observation group(60 cases, nifedipine and atosiban)by random number table method. The clinical efficacy, placental blood perfusion related indicators, perinatal indicators,maternal-infant outcomes and adverse reactions were observed in both groups. Results The total clinical effective rate in the observation group after treatment was 98.33%,which was higher than 86.67%in the control group(P<0.05).Before treatment, there was no statistical difference in vascularization flow index(VFI),blood flow index(FI)and placental vascularization index(VI)between the 2 groups. After treatment, the VFI,FI and VI of patients in the observation group were lower(P<0.05).The duration of pregnancy extension, neonatal Apgar(appearance, pulse, grimace, activity, and respiration)score at 1 minute after birth and lecithin/sphingomyelin ratio in the observation group were higher(P<0.05),and the time of uterine contraction disappearance was shorter(P<0.05).The incidence rates of fetal distress,meconium-stained amniotic fluid and intrauterine infection in the observation group were lower when compared with those in the control group(P<0.05).The incidence rates of adverse reactions of tachycardia, headache, chest tightness and hypokalemia in the observation group were not statistically different from those in the control group(P>0.05).Conclusion Atosiban combined with nifedipine has a definite efficacy in the threatened preterm premature rupture of membranes, and it can stabilize placental blood perfusion, improve body mass, promote infant lung maturation and improve infant outcome, with high safety.
