Establishment of a Nomogram prediction model for psychological distress in elderly primiparas based on perceived stress,family intimacy and adaptability
Objective To explore the relationships of psychological distress with perceived stress,family cohesion and adaptability in the elderly primiparas,and establish a Nomogram model for prediction of psychological distress.Methods Convenience sampling was employed to select 263 elderly primiparas(≥35 years old)who received regular prenatal examinations and gave birth in Xiaolan People's Hospital from January 2020 to December 2022 as the survey objects.General information questionnaire,the 10-item Kessler psychological distress scale(Kessler 10),Chinese perceived stress scale(CPSS)and family adaptability and cohesion scale(FACESⅡ-CV)were used in the survey in the third trimester of pregnancy.Multiple logistic regression was used to analyse the influencing factors of psychological distress in the elderly primiparas.The Nomogram prediction model was established based on the results of regression analysis.Receiver operating characteristic(ROC)curve was adopted to assess the effectiveness of the prediction model.Results The Kessler 10 score of the 256 elderly primiparas was(19.49±5.58),with 105(41.02%)elderly primiparas had psychological distress.The Kessler 10 score was positively correlated with the scores of tension and sense of loss of control as well as the total score of CPSS(P<0.05),and it was negatively correlated with the scores of family cohesion and adaptability as well as the total score of FACESⅡ-CV(P<0.05).Multiple logistic regression analysis showed that the education with junior school or lower,per capita monthly income of the household<3000 RMB,pregnant comorbidity,pregnancy complications and CPSS score were the risk factors for psychological distress(P<0.05).Meanwhile,the FACESⅡ-CV score was a protective factor(P<0.05).The Nomogram model showed that the area under the curve(AUC)was 0.927,with a sensitivity at 91.4%and a specificity at 84.1%.Conclusions Psychological distress of the elderly primiparas in third trimester of pregnancy is related to perceived stress,family cohesion and adaptability.The Nomogram prediction model is highly valuable for predicting psychological distress in the elderly primiparas.
