脑钠肽及尿素氮与急性上消化道出血并发心肌损伤的相关性研究

摘要
目的 探讨脑钠肽(BNP)、血尿素氮(BUN)水平与急性上消化道出血(AUGIB)患者并发心肌损伤(MD)的相关性.方法 选择 2020年 2月至2022年 2月在浙江省荣军医院接受治疗的AUGIB患者 231例,根据是否发生MD分为MD组与非MD组;另择同期本院健康体检者 100名为对照组.测定血清BNP及BUN水平.采用Spearman秩相关探究BNP、BUN水平与AUGIB患者MD的相关性,采用ROC曲线评估BNP、BUN水平对AUGIB患者并发MD的预测效能.结果 MD组、非MD组BNP、BUN水平高于对照组,MD组BNP、BUN水平高于非MD组,差异均有统计学意义(均P<0.01).Spearman秩相关分析显示,BNP、BUN水平与AUGIB患者并发MD均呈正相关(rs=0.648、0.591,均P<0.01).多因素logistic分析结果显示,BNP、BUN水平越高,AUGIB患者并发MD的风险越高(均P<0.01).ROC曲线分析显示,BNP与BUN两者联合预测AUGIB并发MD的AUC高于BNP与BUN单独预测的AUC(均P<0.01).结论 BNP、BUN增高的AUGIB患者发生MD的风险增大.
Abstract
Objective To investigate the correlation of brain natriuretic peptide(BNP)and blood urea nitrogen(BUN)with myocardial damage(MD)after acute upper gastrointestinal bleeding(AUGIB).Methods A total of 231 AUGIB patients who received treatment in Zhejiang Rongjun Hospital from February 2020 to February 2022 were selected and divided into MD group and non-MD group according to whether MD occurred.Another 100 healthy subjects were selected as a control group.Serum BNP and BUN were measured.Spearman analysis was used to explore the association of BNP and BUN with MD in AUGIB patients.ROC curve was used to evaluate the predictive effect of BNP and BUN levels on MD occurrence after AUGIB.Results BNP and BUN were significantly higher in MD group and non-MD group than in the control group(all P<0.05).BNP and BUN in MD group were significantly higher than those in non-MD group(all P<0.05).Spearman correlation analysis showed that BNP and BUN were positively correlated with MD in AUGIB patients(rs=0.648,0.591,both P<0.01).Multivariate logistic analysis showed that the higher the levels of BNP and BUN,the higher the risk of MD in AUGIB patients(all P<0.05).ROC curve analysis showed that the combined AUC of BNP and BUN was significantly higher than the individual AUC of BNP and BUN(both P<0.01).Conclusion The risk of MD increases in AUGIB patients with increased BNP and BUN.
