摘要
目的 探讨血清可溶性生长刺激表达基因2蛋白(sST2)在急性ST段抬高心肌梗死(STEMI)患者中的表达及对预后的影响.方法 回顾性选取2020年1月至2022年4月在金华市中心医院心内科住院治疗的急性STEMI患者142例(STEMI组),另择非冠心病及心力衰竭患者72例作为对照(对照组).采用酶联免疫吸附法测定并比较两组血清sST2水平.观察1年内的主要不良心血管事件(MACE),并分为MACE组和非MACE组,分析MACE的危险因素及sST2对MACE的预测价值.结果 STEMI 组 sST2 水平 36.80(19.31,66.47)ng/mL,高于对照组的 12.20(9.11,15.66)ng/mL(P＜0.001);二元 logistic 回归显示 sST2 是 MACE 发生的危险因素(OR=1.032,95％CI 1.001～1.063,P=0.040),sST2 预测 1 年内发生MACE的AUC为0.815,截断值为45.969 ng/mL,灵敏度为0.844,特异度为0.736.结论 sST2与STEMI发病相关,并可预测STEMI患者1年内的MACE.
Abstract
Objective To investigate the expression of soluble growth stimulation expressed gene 2 protein(sST2)in patients with acute ST segment elevation myocardial infarction(STEMI)and its impact on prognosis.Methods 142 patients with acute STEMI who were hospitalized in the cardiology department of Jinhua Municipal Central Hospital from January 2020 to April 2022 were selected as STEMI group,while 72 patients without coronary heart disease and heart failure were selected as a control group.Enzyme linked immunosorbent assay was used to measure serum sST2.Major adverse cardiovascular events(MACE)were observed during one year follow-up,and STEMI patients were divided into MACE group and non-MACE group.The risk factors and the predictive value of sST2 for MACE were analyzed.Results The sST2 was 36.80(19.31,66.47)ng/mL in STEMI group,significantly higher than 12.20(9.11,15.66)ng/mL in the control group(P＜0.001).Binary logistic regression showed that sST2 was a risk factor for MACE(OR=1.032,95％CI 1.001-1.063,P=0.040).The AUC of sST2 for predicting MACE within one year was 0.815,with cutoff value of 45.969 ng/mL,sensitivity of 0.844,and specificity of 0.736.Conclusion sST2 is associated with STEMI and can predict MACE in STEMI patients within one year.
