Objective To explore the characteristics of cerebral blood flow perfusion and their clinical significance in young bipolar disorder(BD)typeⅡ patients with suicidal ideation.Methods A retrospective study was conducted included 84 first-time BD type Ⅱ patients,aged 17-40 years who were admitted to the Department of Psychiatry at Wenzhou Seventh People's Hospital from February 2020 to April 2021.According to the total score of the three factors(despair,optimism,and sleep)of the self-rating scale for suicide intention(SIOSS),they were divided into a suicidal ideation group(≥12 points)with 40 cases and a non-suicidal ideation group(<12 points)with 44 cases.Another 40 healthy individuals who underwent physical examinations in our hospital during the same period were selected as the control group.MRI was used to detect resting cerebral blood flow(CBF),and the Montreal cognitive assessment scale(MoCA),Hamilton anxiety scale(HAMA),24-item Hamilton depression scale(HAMD24),SIOSS scores,and local CBF were compared among three groups of study subjects.The correlation between CBF in different brain regions and scores on each scale was analyzed.Results The total MoCA scores of the group with suicidal ideation and the group without suicidal ideation were significantly lower than those of the control group(both P<0.05),and the group with suicidal ideation was lower than the group without suicidal ideation(P<0.05).HAMA in the group with suicidal ideation,HAMA in the group without suicidal ideation The HAMD24 and SIOSS scores were significantly higher than the control group(all P<0.05),and the group with suicidal ideation was higher than the group without suicidal ideation(all P<0.05).The CBF in the left putamen,left anterior cingulate cortex,and left superior temporal gyrus of the group with suicidal ideation was significantly higher than that of the group without suicidal ideation and the control group(all P<0.05),while the CBF in the right posterior cingulate gyrus and right inferior temporal gyrus was lower than that of the group without suicidal ideation(all P<0.05).Left putamen,left superior temporal gyrus,and right inferior temporal gyrus CBF and MoCA,HAMA,HAMD24,and SIOSS scores were all positively correlated(all P<0.05).Left anterior cingulate cortex,right posterior cingulate cortex CBF and MoCA HAMA,HAMD24,and SIOSS scores were all negatively correlated(all P<0.05).Conclusion In young BD type Ⅱ patients with suicidal ideation,the CBF in the left putamen,left anterior cingulate cortex,and left superior temporal gyrus of is elevated,while the CBF in the right posterior cingulate gyrus and right inferior temporal gyrus is significantly reduced,and is associated with cognitive function,anxiety symptoms,depression symptoms,and suicidal ideation.
