摘要
目的 检索、评价并总结预防老年下肢骨折术后发生深静脉血栓的最佳证据,为临床工作提供循证依据.方法 确立循证问题,依据"6S"证据模型,自上而下进行计算机检索证据.包括系统评价、临床指南、随机对照试验、证据总结、专家共识,检索时限为建库至2022年12月25日.由2名研究者对文献质量进行评价和证据提取、汇总,并对纳入文献评定证据级别.结果 共纳入 13 篇文献,包括指南 2 篇,专家共识 6 篇,系统评价 4 篇,随机对照试验 1 篇.汇总了包含风险评估、基础预防、物理预防、药物预防、健康教育 5 个方面的 20 条证据.结论 该研究针对预防老年下肢骨折术后发生深静脉血栓进行证据总结,可为护理人员开展临床实践提供参考依据.
Abstract
Objective To retrieve,evaluate and summarize the best evidence for the prevention of deep vein thrombosis after lower limb fracture in the elderly and provide an evidence-based basis for clinical work.Methods Evidence-based questions were established and a top-down computerized search was conducted based on the"S"evidence model.It included clinical decision making,systematic evaluation,clinical guidelines,randomized controlled trials,evidence summaries,and expert consensus,and the search time frame was built up to December.The quality of the literature was evaluated and evidence extracted and summarised by investigators,and the level of evidence was assigned to the included literature.Results A total of publications were included,including guidelines,expert consensus,systematic reviews,and randomised controlled trial.Twenty pieces of evidence containing five aspects of risk assessment,basic prevention,physical prevention,pharmacological prevention,and health education were pooled.Conclusion This study provides a summary of evidence for the prevention of deep vein thrombosis after lower limb fracture surgery in the elderly,which can provide a reference for nursing staff to carry out clinical practice.
