农村老年糖尿病低血糖昏迷患者健康自我管理体验的访谈式调查分析

摘要
目的 深入了解农村老年糖尿病低血糖昏迷患者健康自我管理的体验,探讨其面临的问题及解决方案,为后续研究提供理论基础.方法 选取 2022 年 1-12 月医院急诊中心救治的 12 例糖尿病低血糖昏迷患者为调查对象.采用访谈式质性研究方法进行面对面、半结构化访谈.采用Colaizzi七步分析法进行提炼分析主题.结果 共提炼出3个主题与8个亚主题:认知行为矛盾(疾病危害认知受阻、健康促进内驱力不足)、医疗决策困难(医疗资源薄弱、就医决策障碍、就医体验不良)、社会支持缺失(自我管理行为消极、呼唤情亲照料、延伸服务需求).结论 目前农村老年糖尿病低血糖昏迷患者健康管理现状不佳,社会家庭支持缺失,健康自我管理对提升农村老年糖尿病患者生活质量具有积极意义,需采取有效方法促进农村老年糖尿病患者的健康管理能力.
Abstract
Objective To deeply understand the experience of health self-management of elderly patients with diabetes and hypoglycemia in rural coma,explore their problems and solutions,and provide a theoretical basis for subsequent research.Methods Twelve diabetic hypoglycemic coma patients treated in the emergency center of our hospital from January to December 2022 were selected as research objects by objective sampling method.Use descriptive research methods,through face-to-face,semi-structured interviews.Colaizzi's seven-step analysis method was used to further refine and analyze the topic of the interview data.Results Through analysis and summary,3 themes and 8 subthemes were extracted:cognitive behavioral contradiction(impaired disease hazard cognition,insufficient health promotion drive),difficulty in medical decision-making(weak medical resources,decision-making obstacles,poor medical experience),lack of social support(negative self-management behavior calls for extended services of loving parents).Conclusion At present,the health management of elderly patients with diabetes and hypoglycemia in coma in rural areas is not good,and the lack of social and family support.Healthy self-management has positive significance for improving the quality of life of elderly patients with diabetes in rural areas,and effective methods should be taken to promote the health management ability of elderly patients with diabetes in rural areas.
