摘要
目的 采用临床病例对照研究,分析乳腺癌患者化疗致周围神经病变的危险因素,为临床开展乳腺癌患者化疗致周围神经病变的防控工作提供理论依据.方法 收集2022年8月-2023年6月唐山市人民医院乳腺外科化疗期间发生周围神经病变的 270 例乳腺癌患者为病例组,并选取同时期化疗期间未发生周围神经病变的 270 例乳腺癌患者为对照组.收集两组患者临床资料,通过单因素分析及多因素Logistic回归模型,探讨乳腺癌患者化疗致周围神经病变的危险因素.结果 单因素分析显示,年龄、身体质量指数、化疗药物输注时间、有无固定照顾者、高血压、化疗药物累计剂量与发生周围神经病变有关(P<0.05);缺乏维生素D、谷丙转氨酶、谷草转氨酶、化疗前贫血与发生周围神经病变有关(P<0.05);多因素Logistic回归分析显示,年龄、高血压病史、化疗药物累计剂量、缺乏维生素D、谷丙转氨酶、化疗前贫血、化疗药物输注时间、身体质量指数为乳腺癌患者化疗致周围神经病变的危险因素(P<0.05);有固定照顾者为乳腺癌患者化疗致周围神经病变的保护因素(P<0.05).结论 年龄、有高血压病史、化疗药物累计剂量＞400mg、缺乏维生素D、谷丙转氨酶异常、化疗前贫血、化疗药物输注时间＞30 分钟、身体质量指数≥24、无固定照顾者为乳腺癌患者化疗致周围神经病变的危险因素.
Abstract
Objective To analyze the risk factors of chemotherapy induced peripheral neuropathy in breast cancer patients by clinical case-control study,so as to provide a theoretical basis for the prevention and control of chemotherapy induced peripheral neuropathy in breast cancer patients.Methods From August 2022 to June 2023,270 patients with breast cancer who suffered from peripheral neuropathy during chemotherapy in the breast surgery department of Tangshan People's Hospital were collected as the case group,and 270 patients with breast cancer who did not suffer from peripheral neuropathy during chemotherapy at the same time were selected as the control group.The clinical data of the two groups of patients were collected,and the risk factors of peripheral neuropathy caused by chemotherapy in breast cancer patients were explored through single factor analysis and multiple factor logistic regression model.Results Single factor analysis showed that age,body mass index,duration of chemotherapy drug infusion,presence or absence of fixed caregivers,hypertension,and cumulative dose of chemotherapy drugs were associated with the occurrence of peripheral neuropathy(P<0.05);Lack of vitamin D,alanine aminotransferase,aspartate aminotransferase,and pre chemotherapy anemia were associated with the occurrence of peripheral neuropathy(P<0.05);Multivariate logistic regression analysis showed that age,hypertension history,cumulative dose of chemotherapy drugs,lack of vitamin D,alanine aminotransferase,anemia before chemotherapy,infusion time of chemotherapy drugs,body mass index were risk factors for peripheral neuropathy caused by chemotherapy in breast cancer patients(P<0.05);Fixed caregivers were protective factors of peripheral neuropathy caused by chemotherapy in breast cancer patients(P<0.05).Conclusion Age,history of hypertension,cumulative dose of chemotherapy drugs>400mg,lack of vitamin D,abnormal alanine aminotransferase,anemia before chemotherapy,infusion time of chemotherapy drugs>30 minutes,body mass index≥24,and no fixed caregivers are risk factors for peripheral neuropathy caused by chemotherapy in breast cancer patients.
