单侧双通道内镜和单通道内镜下腰椎融合手术治疗腰椎退行性疾病的临床效果对比分析

摘要
目的 对比分析单侧双通道内镜(UBE)和单通道内镜下腰椎融合手术(Endo-TLIF)对腰椎退行性疾病(LDD)患者的疗效.方法 以 2022 年 10 月-2023 年 10 月医院收治的LDD患者为研究对象.选择其中采用UBE治疗的 31例患者为观察组(UBE组),以组间匹配的方式选择采用Endo-TLIF治疗的29例为对照组(Endo-TLIF组).比较两组患者的手术情况、疼痛程度、腰痛功能障碍及并发症发生率.结果 UBE治疗后,观察组患者术中出血量、手术时间、住院时间均优于对照组,差异有统计学意义(P<0.05).治疗前,两组患者疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数问卷表(ODI)比较,差异无统计学意义(P>0.05);治疗后1个月,两组患者VAS评分、ODI评分都降低,但两组患者VAS评分、ODI评分比较,差异仍无统计学意义(P>0.05).UBE治疗后,观察组患者并发症发生率(12.90%)与对照组患者并发症发生率(10.34%)比较,差异无统计学意义(P>0.05).结论 UBE和Endo-TLIF治疗LDD患者均可获得较好的临床疗效,与Endo-TLIF相比,UBE具有术中出血量减少,手术时间、住院时间缩短等优势.
Abstract
Objective To investigate the effects of unilateral two-channel endoscopy(UBE)and single-channel endoscopic lumbar fusion surgery(Endo-TLIF)on the efficacy and complications of patients with lumbar degenerative disease(LDD).Methods Sixty patients with LDD admitted to the hospital from October 2022 to October 2023 were selected as the study subjects.According to the different surgical methods,31 patients were divided into the UBE group(observation group),and 29 patients were included in the Endo-TLIF group(control group).Surgical status,pain level,LP dysfunction,and incidence of complications were compared between the two groups.Results After UBE treatment,the intraoperative blood loss,operation time and hospital stay exceeded the control group(P<0.05).Before treatment,VAS and ODI scores were compared(P>0.05);one month after treatment,both VAS and ODI scores decreased,but VAS and ODI scores were compared between the two groups(P>0.05).After UBE,the observed patients(12.90%)and control patients(10.34%)(P>0.05).Conclusion Both UBE and Endo-TLIF can treat LDD patients,and compared with Endo-TLIF,UBE has the advantages of reducing intraoperative blood loss and shorter operation time and hospital stay.
