非药物干预措施在预防重症监护室患者谵妄中的应用效果

摘要
目的 探讨在重症监护病房(ICU)中使用眼罩和耳塞预防谵妄的效果.方法 选择2023年5月1日-11月 10 日医院ICU(包括综合重症监护室、冠心病重症监护室、心外科重症监护室)住院的 60 例患者作为研究对象,根据组间基线资料均衡可比的原则,采用随机数字表法将患者分为观察组和对照组,各 30 例.观察组患者提供了 3个晚上的口罩和耳塞;对照组无任何干预措施.结果 干预后,观察组和对照组患者在第1天及第2天白天谵妄的发生率差异无统计学意义(P>0.05);第2天夜间及第3天白天和夜间,对照组患者的谵妄发生率明显高于观察组,组间差异均有统计学意义(P<0.05).观察组患者在3个晚间的睡眠质量明显优于对照组,组间睡眠质量平均得分差异有统计学意义(P<0.05).结论 重症监护患者隔夜使用的耳塞和眼罩可有效提高睡眠质量和预防谵妄.
Abstract
Objective To explore the effectiveness of using eye masks and earplugs in the intensive care unit(ICU)to prevent delirium.Methods Sixty patients admitted to the ICU of the hospital(including the comprehensive intensive care unit,coronary heart disease intensive care unit,and cardiac surgery intensive care unit)from May 1,2023 to November 10,2023 were selected as the study subjects.Based on the principle of balanced and comparable baseline data between groups,the patients were randomly divided into an observation group and a control group,with 30 patients in each group,using a random number table method.The observation group patients were provided with masks and earplugs for three nights;There were no intervention measures in the control group.Results After the intervention,there was no statistically significant difference in the incidence of daytime delirium between the observation group and the control group on the first and second day(P>0.05);The incidence of delirium in the control group was significantly higher than that in the observation group on the second night and the third day during the day and night,with statistically significant differences between the groups(P<0.05).The sleep quality of the observation group patients was significantly better than that of the control group in three nights,and the average score difference of sleep quality between the groups was statistically significant(P<0.05).Conclusion Overnight use of earplugs and eye masks in intensive care patients can effectively improve sleep quality and prevent delirium.
