摘要
目的 探究基于加速康复理念的康复管理对创伤性肱骨骨折患者的疼痛水平以及睡眠质量的影响.方法 选取2021年6月-2023年6月医院收治的180例创伤性肱骨骨折患者为研究对象,按照组间性别、年龄、体重指数、骨折部位、骨折类型均衡可比的原则分为对照组和观察组,各 90 例.对照组采取常规康复管理,观察组在对照组基础上采取基于加速康复理念的康复管理.比较两组患者的视觉模拟量表(VAS)疼痛评分、肩关节功能(Constant-Murley)评分、肘关节功能(Mayoo)评分、睡眠质量以及并发症情况.结果 康复管理干预前,两组患者VAS、Constant-Murley、Mayoo评分比较,差异无统计学意义(P>0.05);康复管理后,两组患者VAS评分都降低,Constant-Murley、Mayoo评分都升高,但观察组患者VAS低于对照组,而Constant-Murley、Mayoo评分高于对照组,组间差异均优于对照组,差异有统计学意义(P<0.05).采用基于加速康复理念的康复管理后,观察组患者PSQI总分及各项睡眠因子得分、术后并发症发生率均低于对照组,差异有统计学意义(P<0.05).结论 对于创伤性肱骨骨折患者采取基于加速康复理念的康复管理模式,能有效减轻患者疼痛水平,改善肩关节和肘关节功能,提高其睡眠质量,降低并发症的发生.
Abstract
Objective To investigate the effect of rehabilitation management based on the concept of accelerated rehabilitation on the pain level as well as sleep quality of patients with traumatic humerus fracture.Methods A total of 180 traumatic humeral fracture patients admitted to the hospital from June 2021 to June 2023 were selected as the study subjects,and were divided into control group and observation group according to the principle of comparable sex,age,BMI,fracture site and fracture type,with 90 cases each.The control group adopted routine rehabilitation management,and the observation group adopted rehabilitation management based on the concept of accelerated rehabilitation on the basis of the control group.The visual analogue scale(VAS)pain score,shoulder function(Constant-Murley)score,elbow function(Mayoo)score,sleep quality,and complications were compared between the two groups.Results Before the intervention,VAS,Constant-Murley and Mayoo scores were not significantly different(P>0.05).After rehabilitation management based on accelerated rehabilitation,VAS scores decreased,Constant-Murley and Mayoo scores increased,but in the observation group,VAS,Constant-Murley and Mayoo scores were better than those of the control group,significantly(P<0.05).After rehabilitation management based on the concept of accelerated rehabilitation,the scores of all sleep factors,total scores of PSQI and postoperative complications of the patients in the observation group were lower than that of the control group,and there was a statistically significant difference(P<0.05).Conclusion The rehabilitation management mode based on the adoption of the concept of accelerated rehabilitation can effectively reduce the pain level,improve the function of shoulder and elbow joints,improve their sleep quality,and reduce the occurrence of complications.
