虚拟现实技术联合重复经颅磁刺激治疗脑卒中后抑郁的临床效果

摘要
目的 探讨虚拟现实技术联合重复经颅磁刺激治疗对脑卒中后抑郁患者的临床效果.方法 选取2022年1月-2023年12月在遵义医科大学第五附属(珠海)医院住院的脑卒中后抑郁患者64例为研究对象.采用随机数字表法,结合组间基线特征匹配的原则,分为观察组和对照组,每组各32例.对照组患者采用重复经颅磁刺激治疗,观察组患者在重复经颅磁刺激治疗的基础上应用虚拟现实技术治疗,疗程均为4周.比较两组患者的汉密尔顿抑郁量表(HAMD)评分、美国国立卫生研究院卒中量表(NIHSS)评分、改良Barthel指数评分及健康调查量表(SF-36)评分.结果 治疗前,两组患者的HAMD量表的评分、NIHSS评分、改良Barthel指数评分、SF-36评分比较,差异均无统计学意义(P>0.05).治疗4周后,观察组患者的HAMD量表的评分、NIHSS评分均低于对照组,差异均有统计学意义(P<0.05):而观察组患者的改良Barthel指数评分、治疗总有效率均高于对照组,差异均有统计学意义(P<0.05).两组患者治疗4周后的SF-36评分比较,差异无统计学意义(P>0.05).结论 虚拟现实技术联合重复经颅磁刺激治疗可以改善脑卒中后患者的抑郁程度,同时改善脑卒中后神经功能损害的程度和日常生活能力,疗效均优于单纯重复经颅磁刺激治疗.
Abstract
Objective To explore the clinical effect of virtual reality technology combined with repetitive transcranial magnetic stimulation on patients with post-stroke depression.Methods 64 patients with post-stroke depression hospitalized in the Fifth Affiliated Hospital of Zunyi Medical University(Zhuhai)from January 2022 to December 2023 were selected as the study objects.According to the principle of matching baseline characteristics between groups,the patients were divided into observation group and control group by random number table method,with 32 cases in each group.Patients in the control group were treated with repeated TMS,and patients in the observation group were treated with virtual reality technology on the basis of repeated TMS.The treatment course was 4 weeks.The Hamilton Depression Scale(HAMD)score,National Institutes of Health Stroke Scale(NIHSS)score,modified Barthel Index score,and Health Survey Scale(SF-36)score were compared between the two groups.Results There was no significant difference in HAMD scale score,NIHSS score,modified Barthel index score and SF-36 score between the two groups before treatment(P>0.05).After 4 weeks of treatment,the HAMD score of the observation group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).The scores of HAMD and NIHSS in the observation group was lower than that of the control group after 4 weeks of treatment,and the difference was statistically significant(P<0.05).After 4 weeks of treatment,The scores of modified Barthel index and the total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in SF-36 scores between the two groups after 4 weeks of treatment(P>0.05).Conclusion Virtual reality technology combined with repeated TMS therapy can improve the degree of depression of patients after stroke,and improve the degree of neurological impairment and daily living ability after stroke,and the efficacy is better than repeated TMS therapy alone.
