痛泻要方加味联合针灸治疗肠易激综合征有效性的Meta分析
Meta-analysis of Tongxie yaofang combined with acupuncture in the treatment of irritable bowel syndrome

摘要
目的 系统评价痛泻要方加味联合针灸治疗肠易激综合征(irritable bowel syndrome,IBS)的随机对照试验的临床疗效.方法 计算机全面检索自建库起至 2023 年 2 月 24 日中国知网(CNKI)、万方数据库(Wanfang Database)、维普数据库(VIP)、中国生物医学文献数据库(CBM)、PubMed、Web of science、Embase和The Cochrane Library各数据库中痛泻要方加味联合针灸治疗肠易激综合征的临床随机对照试验(RCTs),采用RevMan5.3软件对纳入研究进行资料提取、数据合并、质量评价及Meta分析.结果 本次研究纳入15篇文献,共计1295例患者.Meta分析结果显示,观察组临床疗效优于对照组(RR=1.300,95%CI:1.290,1.370);痛泻要方加味联合针灸可以改善腹痛症状积分(MD=-0.770,95%CI:-1.180,-0.350)、腹胀症状积分(MD=-0.650,95%CI:-1.160,-0.140);观察组的血清降钙素基因相关肽(CGRP)(MD=-3.550,95%CI:-3.970,-3.130)、血管活性肽(VIP)(MD=-1.110,95%CI:-2,100,-0.120)低于对照组;观察组的复发率低于对照组(RR=0.250,95%CI:0.140,0.430).结论 痛泻要方加味联合针灸治疗IBS具有良好的临床疗效,且明显优于西药治疗,但仍然需要高质量的RCT试验进一步验证.
Abstract
Objective To systematically evaluate the clinical efficacy of Tongxie Yaofang combined with acupuncture in the treatment of irritable bowel syndrome in a randomized controlled trial.Methods A comprehensive search was conducted on CNKI,Wanfang Data,VIP,Chinese biomedical literature Database(CBM),Pubmed,Web of science,Embase and The Cochrane Library.The retrieval time is from the self-established database to February 24,2023.The related literatures of randomized controlled trials(RCTs)based on Tongxie Yaofang combined with acupuncture in the treatment of irritable bowel syndrome were collected,data extraction and combination were performed with Revman5.3 software to analyze the data by meta-analysis.Results Fifteen literatures were included in this study,with a total of 1295 patients.The results of Meta-analysis showed that the clinical efficacy of the observation group was better than that of the control group[RR=1.300,95%CI[1.290,1.370](P<0.00001)].Tongxie Yaofang combined with acupuncture can improve abdominal pain symptom score[MD=-0.770,95%CI(-1.180,-0.350),P=0.0003]and abdominal distension symptom score[MD=-0.650,95%CI(-1.160,-0.140),P=0.01].Serum calcitonin gene-related peptide(CGRP)[MD=-3.550,95%CI(-3.970,-3.130),P<0.00001]and vasoactive peptide(VIP)[MD=-1.110,95%CI(-2,100,-0.120),P=0.03]decreased significantly after treatment compared with the control group.It also effectively reduced the recurrence rate[RR=0.250,95%CI(0.140,0.430),P<0.00001].Conclusion Tongxie yaofang combined with acupuncture has good clinical efficacy in the treatment of IBS,and is significantly better than western medicine treatment,but it still needs to be further verified by high-quality RCT trials.
