早期下地活动对面肌痉挛微血管减压术后康复的影响
Impact of early mobilization on rehabilitation after microvascular decompression surgery for facial spasm

摘要
目的 探讨早期下地活动对接受微血管减压术(microvascular decompression,MVD)的面肌痉挛患者术后康复效果的影响.方法 选取2023年1-5月入住北京大学人民医院神经外科拟行MVD的228例面肌痉挛患者,采用随机数字表法将其分为观察组和对照组,各114例.观察组患者术后48小时内下地活动,而对照组患者术后48小时后下地活动.比较两组患者术后眩晕、疼痛的视觉模拟评分(visual analogue scale,VAS)、术后住院时间以及术后并发症的发生率.结果 两组患者的年龄、饮酒史、冠心病史、病变部位、病程比较,差异均有统计学意义(P<0.05).早期下地活动实施后,观察组患者术后眩晕和疼痛的VAS评分均显著低于对照组,住院时间短于对照组,差异有统计学意义(P<0.05).观察组患者的术后并发症总体发生率为18.4%,显著低于对照组的52.6%,差异有统计学意义(P<0.05).结论 早期下地活动可以减少面肌痉挛患者MVD后的眩晕、疼痛,缩短住院时间,并降低并发症率,促进患者术后康复.
Abstract
Objective To investigate the effect of early ground activity on the postoperative rehabilitation of patients with hemifacial spasm undergoing microvascular decompression(MVD).Methods A total of 228 patients with hemifacial spasm who were admitted to the Neurosurgery Department of Peking University People's Hospital for MVD from January to May 2023 were selected and randomly divided into an observation group and a control group,with 114 cases in each group,using a random number table method.Patients in the observation group should be active on the ground within 48 hours after surgery,while patients in the control group should be active on the ground 48 hours after surgery.Compared the visual analogue scale(VAS),length of hospital stay,and incidence of postoperative complications of postoperative dizziness and pain between two groups of patients.Results There were statistically significant differences in age,alcohol consumption history,coronary heart disease history,lesion location,and disease duration between the two groups of patients(P<0.05).After the implementation of early ground activities,the VAS scores of postoperative dizziness and pain in the observation group were significantly lower than those in the control group,and the hospitalization time was shorter than that in the control group,with statistical significance(P<0.05).The overall incidence of postoperative complications in the observation group was 18.4%,significantly lower than the control group's 52.6%,and the difference was statistically significant(P<0.05).Conclusion Early mobilization can reduce dizziness and pain after MVD in patients with hemifacial spasm,shorten hospitalization time,reduce complication rates,and promote postoperative recovery.
