脑卒中偏瘫患者配偶照顾负担对二元应对的影响及婚姻调适的中介作用

摘要
目的 探讨婚姻调适在脑卒中偏瘫患者配偶照顾负担与二元应对间的中介作用,为提高配偶二元应对水平提供理论参考.方法 选取华北理工大学附属医院、开滦总医院神经内、外科的脑卒中患者的配偶为调查对象,采用一般情况调查表、照顾负担量表(ZBI)、二元应对量表(DCI)、婚姻调适量表(MAT)进行调查.应用SPSS 25.0与AMOS 23.0对数据进行统计分析.结果 ZBI得分59.49±13.90分,DCI得分86.03±16.62分,MAT得分88.39±20.463分.照顾负担与二元应对呈负相关(r=-0.790,P＜0.01),与配偶婚姻调适水平呈负相关(r=-0.530,P＜0.01);二元应对与配偶婚姻调适水平呈正相关(r=0.636,P＜0.01).Bootstrap法验证结果显示,配偶婚姻调适水平在配偶照顾负担与二元应对间存在中介效应,中介效应占总效应的20％.结论 婚姻调适是脑卒中偏瘫患者配偶照顾负担与二元应对间的中介变量.医护人员应尝试帮助其加强沟通交流,改善双方亲密关系,提高配偶婚姻调适水平,提高脑卒中偏瘫患者配偶的二元应对水平,减轻其照顾负担,促进脑卒中患者的康复.
Abstract
Objective To explore the mediating role of spouse marriage adjustment between spouse care burden and spouse's dual coping in stroke patients with hemiplegia,and to provide theoretical reference for improving spouse's dual coping.Methods The spouses of stroke patients in neurology and neurosurgery of a 3A hospital were selected by convenient sampling,and the general situation questionnaire,care burden scale(ZBI),binary coping scale(DCI)and marital adjustment scale(MAT)were used to investigate.SPSS 25.0 and AMOS 23.0 were used for statistical analysis.Results 311 valid questionnaires were collected,and the effective recovery rate was 86.87％.ZBI score 59.49±13.90,DCI score 86.03±16.62 and MAT score 88.39±20.463.The burden of care is negatively correlated with binary coping(r=-0.790,P＜0.01),and negatively correlated with spouse's marital adjustment level(r=-0.530,P＜0.01);There is a positive correlation between binary coping and marital adjustment level of spouses(r=0.636,P＜0.01).Bootstrap method further verifies the mediation effect of spouse's marriage adjustment level between spouse's care burden and dual coping,and the mediation effect accounts for 20％of the total effect.Conclusion Spouse marriage adjustment is an intermediary variable between spouse care burden and binary coping of stroke hemiplegic patients.Medical staff should try to improve the marital adjustment level of spouses,so as to improve the dual coping level of spouses of stroke patients with hemiplegia,reduce their care burden and promote the rehabilitation of stroke patients.
