液体通气对急性肺损伤猪血气指标影响的meta分析

目的 系统评价液体通气(LV)对急性肺损伤(ALI)猪血气指标的影响。方法 计算机检索万方、知网、维普、PubMed,Embase和 Web of science数据库,查找LV治疗以猪为ALI模型的动物实验,检索时限为建库至2022年2月1日。依照严格的纳入与排除标准,由2位研究者独立地筛选文献、提取资料,评价纳入研究的偏倚风险。采用Review Manager 5。3软件进行meta分析,对连续性资料,氧分压(PO2)、二氧化碳分压(PCO2)、pH值采用均数差(MD)及95%可信区间(95%CI)为疗效分析统计量。结果 最终纳入10个动物实验研究,包括142只模型猪。Meta分析显示:LV能够提高ALI猪1 hPO2[MD=37。77,95%CI 8。41~67。12,P=0。01]、2 hPO2[MD=98。74,95%CI 48。40~149。08,P=0。000 1],4 hPO2[MD=119。93,95%CI 51。42~188。43,P=0。000 6];LV 能够降低 ALI 猪 1 hPCO2[MD=-5。98,95%CI-8。84~-3。12,P<0。000 1];LV 能够提高 ALI 猪 1 h 动脉血 pH 值[MD=0。05,95%CI 0。02~0。07,P<0。000 1]、2 hpH 值[MD=0。07,95%CI 0。05~0。10,P<0。000 01]、4 hpH[MD=0。07,95%CI0。02~0。12,P=0。005]。结论 LV能够提高模型猪ALI后PO2、pH值,降低PCO2,可以改善ALI后的通气状态,为ALI的治疗提供新思路。
Effect of liquid ventilation on blood gas indicators in pigs with acute lung injury:a meta-anlysis
Objective To systematically evaluate the effect of liquid ventilation(LV)on blood gas indi-cators in pigs with acute lung injury(ALI).Methods Wanfang,CNKI,VIP,PubMed,Embase,and Web of Science databases were searched by computer to find animal experiments of LV in the treatment of ALI model in pigs.The retrieval time was up to February 1,2022.According to the strict inclusion and exclusion criteria,two researchers independently screened the literature,extracted the data,and evaluated the risk of bias of the included studies.Meta-analysis was performed using Review Manager 5.3 software.Mean difference(MD)and 95%confidence interval(95%CI)were used for continuous data,oxygen partial pressure(PO2),carbon diox-ide partial pressure(PCO2),and pH value.Results A total of 10 animal studies were included,including 142 model pigs.Meta-analysis showed that LV could increase one hour PO2(MD=37.77,95%CI 8.41-67.12,P=0.01),2 h PO2(MD=98.74,95%CI 48.40-149.08,P=0.000 1),4 h PO2(MD=148.00,95%CI 51.42-188.43,P=0.000 6)in ALI pigs.LV could reduce 1 h PCO2 in ALI model pigs(MD=-5.98,95%CI-8.84--3.12,P<0.000 1).LV could increase arterial blood pH at one hour(MD=0.05,95%CI 0.02-0.07,P<0.000 1),2 h pH(MD=0.07,95%CI 0.05-0.10,P<0.000 01),4 h pH(MD=0.07,95%CI 0.02-0.12,P=0.005).Conclusion LV can improve PO2,pH value and reduce PCO2 after ALI in model pigs,which can improve the ventilation state after ALI and provide new ideas for the treatment of ALI.
