At present,cardiovascular and cerebrovascular diseases are currently one of the chronic dis-eases with the heaviest disease burden in China.With the gradual aging of China's population,the traditional chronic disease management mode of"disease-centered"can no longer meet the health service needs of current residents.The chronic disease management model based on the concept of"active health"is a new type of"health-centered"chronic disease health management model.This mode has established a closer,more conven-ient and affordable communication mode between residents and family doctor teams.It saves the time,man-power and economic cost of family doctor team service,and is conducive to improving the quality and efficiency of chronic disease management and residents'health literacy.Taking cardiovascular and cerebrovascular disea-ses as an example,this paper discusseed the chronic disease management mode and research content based on the concept of"active health"aiming to explore the form and standard of"active health"chronic disease man-agement mode,and provide reference for the promotion of this mode.
