摘要
目的 探讨非酒精性脂肪肝(NAFLD)对老年高血压患者心脏结构和功能的影响.方法 收集2021年 1月至 2023年 12月在该院消化内科、老年科、全科医学科出院的787例年龄大于或等于60岁高血压患者的临床资料,根据住院腹部彩色多普勒超声结果将其分为 NAFLD组(133例)和无 NAFLD组(654例).分析2组患者临床资料和超声心动图检查结果的差异.结果 与无 NAFLD 组相比,NAFLD 组患者左心室后壁厚度、室间隔厚度增厚,左心室质量增大[分别为(11.19±1.21)mm vs.(11.48±1.67)mm、(11.25±1.25)mm vs.(11.52±1.22)mm、(193.20±45.87)g vs.(203.05±48.22)g],差异均有统计学意义(P<0.05);2组患者E/A<1占比比较,差异无统计学意义(P>0.05),但无NAFLD组和NAFLD组患者舒张功能损害占比分别高达74.6%和70.7%.结论 老年高血压NAFLD患者左心室后壁厚度、室间隔厚度、左心室质量均明显高于无NAFLD患者.
Abstract
Objective To investigate the effects of nonalcoholic fatty liver disease(NAFLD)on cardiac structure and function in elderly hypertensive patients.Methods The data of 787 patients with hypertension aged≥60 years from January 2021 to December 2023 in the Department of Gastroenterology,Department of Geriatrics and Department of General Medicine in the hospital were collected,and divided the patients into NAFLD group(133 cases)and the non-NAFLD group(654 cases)according to the results of abdominal ultra-sound.The differences in general clinical data and echocardiography between the two groups were compared.Results Compared with the non-NAFLD group,the left ventricular posterior wall thickness and interventric-ular septal thickness were thicker and left ventricular mass was increased in the NAFLD group,respectively(11.19±1.21)mm vs.(11.48±1.67)mm,(11.25±1.25)mm vs.(11.52±1.22)mm,and(193.20±45.87)g vs.(203.05±48.22)g,and the difference was statistically significant(P<0.05);There was no sig-nificant difference in E/A<1 between the NAFLD group and the non-NAFLD group(P>0.05),but the pro-portion of diastolic dysfunction in the non-NAFLD group and the NAFLD group was as high as 74.6%and 70.7%.Conclusion In elderly hypertensive patients,the interventricular septum thickness,left ventricular posterior wall thickness and left ventricular mass in patients with NAFLD were significantly higher than those without NAFLD.
