Home hospice care practice of a patient with end-stage COPD under the four-way linkage model
To summarize and analyze the practice experience of home hospice care for a patient with end-stage chronic obstructive pulmonary disease under the four-way linkage model involving hospital-commu-nity health services-community-residents.Based on the new model of the four-way linkage model in health care and the establishment of an Internet-based home care platform,the practice promotes physical comfort for the patient through oxygen therapy,rest and activity guidance,dietary advice,and medication guidance.Utili-zing multidisciplinary cooperation,the approach incorporates traditional Chinese medicine music therapy,sup-portive psychological care with empathy,life review with companionship,advance directives,and grief counse-ling to provide spiritual care for both the patient and their family members.After over six months of home care,the patient passed away 7peacefully with their family by their side,and the family members managed to move through the grieving process calmly,achieving peace with life and death,resulting in a satisfactory nurs-ing outcome.
