Therapeutic effect of acupuncture on secondary dysmenorrhea caused by adenomyosis
Objective It is to observe the clinical efficacy of acupuncture in the treatment of dysmenorrhea in adenomy-osis.Methods Seventy-one dysmenorrheal patients with adenomyosis who visited Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine and were recruited through posters from December 2020 to January 2022 were randomly di-vided into 38 cases in the test group and 33 cases in the control group.The experimental group was treated with acupunc-ture,once daily during menstruation and twice per week during non menstruation,the needle remaining time was 30 mi-nutes;the control group was given ibuprofen sustained-release capsules orally during menstruation,one capsule per time,twice daily day,continuously treated for 5 days.Both groups were treated for 3 menstrual cycles.The scores of menstrual worst pain visual analog scale(VAS),pictorial blood loss assessment chart(PBAC),menstrual blood loss chart(PBAC),COX menstrual symptom scale(CMSS),endometriosis health profile-5(EHP-5)before and after treatment,and the incidence of adverse reactions were observed in the two groups.Result The VAS scores of the two groups and the PBAC scores of the experimental group in the 1st,2nd and 3rd menstrual cycles of treatment were significantly lower than those before treatment(all P<0.05);with the prolongation of the treatment cycle,the VAS scores and PBAC scores of the experimental group were gradually reduced,and the differences at each time point were statistically significant after pairwise comparisons(all P<0.05),but the differences at each time point were not statistically significant in the control group(all P>0.05).In the 1st menstrual cycle of treatment,the VAS score of the control group was significantly lower than that of the experimental group(P<0.05);the differences in the VAS scores of the 2nd and3rd menstrual cycles of treatment and the PBAC scores of the 1st,2nd,and 3rd menstrual cycles of treatment were not statistically significant between the two groups(all P>0.05).In the 3rd menstrual cycle of treatment,the CMSS severity and duration scores and EHP-5 scores of the two groups were significantly lower than those before treatment(all P<0.05);the differences in the CMSS severity and duration scores,EHP-5 scores in the 3rd menstrual cycle,and the difference of CMSS severity and duration scores between the 3rd menstrual cycle of treatment and pre-treatment,difference of VAS scores between the 3rd menstrual cycle of treat-ment and pre-treatment were not statistically significant between the two groups(all P>0.05),the difference of PBAC scores and EHP-5 scores between the 3rd menstrual cycle of treatment and pre-treatment in the experimental group were sig-nificantly higher than those in the control group(all P<0.05).Adverse effects in the two groups were mild.Conclusion Acupuncture and ibuprofen capsule are equally effective in reducing the severity of pain and improving the symptoms associ-ated with dysmenorrhea in patients with adenomyosis,but acupuncture may have more advantages in reducing menstruation and improving quality of life.
