电针夹脊穴联合润肌丹油外用治疗糖尿病足溃疡疗效观察

摘要
目的 观察电针夹脊穴联合润肌丹油外用治疗糖尿病足溃疡的临床疗效.方法 选取2022 年8 月—2023 年8 月黑龙江中医药大学附属第一医院收治的糖尿病足溃疡患者60 例,以随机数字表法将其随机分为对照组和治疗组各30 例.对照组予以基础治疗和康复新液纱布换药,治疗组予基础治疗及电针腰部夹脊穴和润肌丹油纱布换药治疗,2 组均以7d为1 个疗程,治疗28d.比较2 组治疗前后创面总面积、踝肱指数(ABI)、血白细胞计数、中医证候积分及临床疗效.结果 与治疗前比较,治疗后2 组患者创面总面积均明显缩小(P 均＜0.05),ABI均明显升高(P 均＜0.05),血白细胞计数及中医证候积分均明显降低(P均＜0.05),治疗组各指标改善情况均明显优于对照组(P均＜0.05).治疗 28d后,治疗组总有效率为 93.3%(28/30),明显高于对照组的73.7%(22/30),差异有统计学意义(P＜0.05).结论 电针夹脊穴联合润肌丹油外用治疗糖尿病足溃疡效果更好,能明显改善肢体缺血,更好地促进创面愈合.
Abstract
Objective It is to observe the clinical efficacy of acupuncture at Jiaji acupoints combined with external ap-plication of Runji pill oil in the treatment of diabetic foot ulcer.Methods Sixty patients with diabetic foot ulcers treated in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from August 2022 to August 2023 were selected and randomly divided into 2 groups by randomized numerical table method:30 cases in the control group and 30 cases in the treatment group.The control group was treated with basic therapy and Kangfuxin liquid by gauze dressing,and the treatment group was treated with basic therapy,acupuncture at Jiaji acupoints and Runji pill oil by gauze dressing,7 days as one treatment course,and both groups were treated for 28 days.The total traumatic area,ankle-brachial index(ABI),white blood cell count,Chinese medicine syndrome scores before and after treatment,and clinical efficacy of the two groups were compared.Results The total traumatic area was significantly reduced,the ABI was significantly increased,the white blood cell count and Chinese medicine syndrome scores were significantly reduced after treatment compared with before treatment in the two groups(all P＜0.05),and the improvements of all indexes in the treatment group were significantly better than those in the control group(all P＜0.05).After 28 days of treatment,the total effective rate of the treatment groupwas93.3%(28/30),which was significantly higher than that of 73.7%(22/30)in the control group,and the difference was statistically significant(P＜0.05).Conclusion Acupuncture at Jiaji acupoints combined with external ap-plication of Runji pill oil are more effective in the treatment of diabetic foot ulcers,which can significantly improve limb is-chemia and better promote wound healing.
