Effect of fast filiform needle acupuncture combined with message on cerebral hemodynamics in patients with cervicogenic vertigo
Objective It is to observe the efficacy of fast filiform needle acupuncture combined with message in the treatment of cervicogenic vertigo and its effect on the hemodynamics of vertebrobasilar artery.Methods Seventy-eight pa-tients with cervicogenic vertigo treated in Lianyungang Affiliated Hospital of Nanjing University of Chinese Medicine from June 2021 to December 2022 were included in the study,and were randomly divided into 38 cases in the experimental group and 40 cases in the control group.The experimental group was treated with fast filiform needle acupuncture combined with message,in which the needle was inserted into the strip like painful nodule from the beginning of the trapezius muscle,and pricked the atlanto-occipital fascia between the occipital bone and atlantoaxial vertebrae once every other day,and al-ternated with the massage therapy,each treatment was carried out for 7 times;the control group was given Flunarizine Hydrochloride Capsules 10 mg orally per night,continuously treated for 14 days.The Evaluation Scale for Cervical Vertigo(ESCV)scores,Dizziness Handicap Inventory(DHI)scores,blood flow velocity of vertebrobasilar artery of patients in the two groups were compared before and after treatment,and the clinical efficacy of the two groups were assessed.Results Compared with before treatment,the ESCV scores of the two groups were significantly increased(both P<0.05),while DHI scores were significantly decreased(all P<0.05),and the improvements of the scores of the experimental group were significantly better than those of the control group(all P<0.05).In the patients of the two groups with accelerated blood flow velocity of vertebrobasilar artery before treatment,the blood flow velocities of left vertebral artery,right vertebral artery and basilar artery were significantly slower after treatment than those before treatment(all P<0.05),and the improve-ments of blood flow velocities of the right vertebral artery and basilar artery of the experimental group were significantly bet-ter than those of the control group(both P<0.05);in the patients of the two groups with slowed blood flow velocity of ver-tebrobasilar artery before treatment,the blood flow velocities of left vertebral artery,right vertebral artery and basilar artery in the experiment group were significantly faster than those before the treatment and in the control group(all P<0.05),no significant changes in the blood flow velocities of left vertebral artery,right vertebral artery and basilar artery were found in the control group(all P>0.05).The total effective rate of the experiment group was 94.7%(36/38),which was signifi-cantly higher than that(85.0%)(34/40)of the control group,and the difference was statistically significant(P<0.05).Conclusion Fast filiform needle acupuncture combined with message can positively regulate the balance function of patients with cervicogenic vertigo,which can achieve the bidirectional regulation of vertebrobasilar artery blood flow,and improve the patients'quality of life,it has some advantages compared with the oral administration of Flunarizine Hydrochlo-ride Capsule.
