核心稳定训练联合穴位敏化针刺治疗盘源性坐骨神经痛的临床研究

目的 观察核心稳定训练联合穴位敏化针刺治疗盘源性坐骨神经痛的临床疗效.方法 选择2021年10月—2023年1月成都医学院第二附属医院·核工业四一六医院康复医学科诊治的盘源性坐骨神经痛患者93例,采用随机数字表法分为对照组、观察1组、观察2组,每组31例.对照组给予常规干预,观察1组在对照组治疗基础上进行核心稳定训练,观察2组在观察1组治疗基础上给予穴位敏化针刺治疗,3组均治疗4周.比较治疗前、治疗4周及治疗结束后2周、4周3组患者疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI),比较治疗前和治疗4周后3组患者血清P物质(SP)、趋化因子配体10(CXCL10)、降钙素相关基因肽(CGRP)水平及弥散张量成像中各向异性分数(FA)、表观弥散系数(ADC)和三维步态分析参数.结果 除对照组治疗结束后4周ODI评分外,3组患者治疗4周、治疗结束后2周和4周的VAS评分和ODI评分均明显低于治疗前(P均<0.05);观察2组治疗4周、治疗结束后2周和4周的VAS评分及治疗4周、治疗结束后2周的ODI评分均明显低于其余2组(P均<0.05).治疗4周后,3组患者血清SP、CXCL10、CGRP水平及ADC均明显低于治疗前(P均<0.05),观察1组和观察2组FA均明显高于治疗前(P均<0.05);且观察2组患者血清CXCL10、CGRP水平及ADC均明显低于其余2组(P均<0.05),血清SP水平明显低于对照组(P<0.05),FA均明显高于其余2组(P均<0.05).治疗4周后,3组步行周期、单支撑时间均明显短于治疗前(P均<0.05),3组步速和观察1组、观察2组步长均明显高于治疗前(P均<0.05);观察2组步速明显高于其余2组(P均<0.05),单支撑时间明显短于其余2组(P均<0.05),步行周期仅短于对照组(P<0.05),步长仅高于对照组(P<0.05).结论 核心稳定训练联合穴位敏化针刺可以明显调节疼痛相关因子水平,缓解盘源性坐骨神经痛患者的疼痛和运动障碍,减轻椎间盘对坐骨神经的压迫,改善步态.
Efficacy of core stabilization training combined with acupoint sensitization acupuncture on discogenic sciatica
Objective It is to observe the clinical efficacy of core stabilization training combined with acupoint sensiti-zation acupuncture in the treatment of discogenic sciatica.Methods Ninety-three patients with discogenic sciatica diagnosed and treated in the Department of Rehabilitation Medicine of the Second Affiliated Hospital of Chengdu Medical College-Nu-clear Industry 416 Hospital from October 2021 to January 2023 were selected and divided into control group,observation I group and observation Ⅱ group by random number table method,with 31 cases in each group.The control group was trea-ted with conventional intervention,the observation Ⅰ group was treated with core stabilization training on the basis of the treatment of the control group,and observation group Ⅱ was treated with acupoint sensitization acupuncture on the basis of the treatment of the observation group Ⅰ.All the three groups were treated for 4 weeks.The pain visual analog scores(VAS)and Oswestry dysfunction index(ODI)of patients in the three groups before treatment,after treatment for 4 weeks,and at 2 and 4 weeks after the end of treatment were compared,and the levels of serum substance P(SP),chemokine lig-and 10(CXCL10),calcitonin gene related peptide(CGRP),fractional anisotropy(FA)and apparent diffusion coefficient(ADC)in DTI and parameters of 3D gait analysis of patients in the three groups before treatment and after treatment for 4 weeks were compared.Results The VAS scores and ODI scores of the patients in the three groups after treatment for 4 weeks,and at 2 and 4 weeks after the end of treatment except for the ODI scores of the control group at 4 weeks after the end of treatment were significantly lower than those before treatment(all P<0.05);the VAS scores after treatment for 4 weeks,and at 2 and 4 weeks after the end of treatment and the ODI scores after treatment for 4 weeks,and at 2 weeks after the end of treatment of the observation Ⅱ group were significantly lower than those of the other two groups(all P<0.05).After 4 weeks of treatment,the serum levels of SP,CXCL10,CGRP and ADC of patients in the three groups were signifi-cantly lower than those before treatment(all P<0.05),and FA in the observation Ⅰ group and observation Ⅱ were sig-nificantly higher than those before treatment(all P<0.05);the serum levels of CXCL10,CGRP and ADC of observationⅡ were significantly lower than those of the other two groups(all P<0.05),with the serum level of SP was significantly lower than that of the control group(P<0.05),FA was significantly higher than those of the other two groups(both P<0.05).After 4 weeks of treatment,the walking cycle and single support time of the three groups were significantly shorter than those before treatment(all P<0.05),the step speed of the three groups and the step length of the observation Ⅰgroup and observation Ⅱ group were significantly higher than those before treatment(all P<0.05);the step speed of the observation Ⅱ group was significantly higher,while the single-support time was significantly shorter than those of the other two groups(all P<0.05),and the walking cycle was only shorter,the step length was only higher than those of the control group(both P<0.05).Conclusion Core stabilization training combined with acupoint sensitization acupuncture can sig-nificantly regulate the levels of pain-related factors,alleviate pain and dyskinesia in patients with discogenic sciatica,re-duce the pressure of intervertebral discs on the sciatic nerve,and improve their gait.
