摘要
目的 观察矾贝散结颗粒联合化疗治疗痰瘀互结证结直肠癌肺转移患者的临床疗效.方法 选取2023年8-12月在烟台市中医医院肿瘤科治疗的84例痰瘀互结证结直肠癌肺转移患者,按随机数字表分为治疗组和对照组各42例,对照组采用mFOLFOX6化疗方案治疗,治疗组在对照组治疗基础上联合矾贝散结颗粒口服,2组均以14 d为1个周期,连续治疗8个周期.比较2组患者肿瘤控制疗效及治疗前后中医证候积分、生活质量[采用生命质量测定量表(EORTC)QLQ-C30(V3.0)与KPS评分评定]、免疫功能(CD4+、CD8+、CD4+/CD8+)和化疗不良反应发生情况.结果 治疗8个周期后,2组患者肿瘤客观缓解率及疾病控制率比较差异均无统计学意义(P均＞0.05);治疗组中医证候积分及EORTC QLQ-C30(V3.0)量表中疲倦、疼痛、气促、食欲丧失、腹泻评分明显低于治疗前及对照组(P均＜0.05),EORTC QLQ-C30(V3.0)量表中躯体功能、角色功能、情绪功能、总体健康状况评分均明显高于治疗前及对照组(P均＜0.05);治疗组KPS评分改善总有效率明显高于对照组[82.05％(32/39)比55.26％(21/38),P＜0.05],且CD4+、CD4+/CD8+均明显高于治疗前及对照组(P均＜0.05),白细胞下降及腹泻发生的程度均明显低于对照组(P均＜0.05).结论 矾贝散结颗粒联合mFOLFOX6化疗方案治疗可明显改善痰瘀互结证结直肠癌肺转移患者的临床症状和机体免疫功能,提高患者生活质量,减轻化疗不良反应.
Abstract
Objective It is to observe the clinical efficacy of Fanbei Sanjie granule in the treatment of patients with lung metastasis of colorectal cancer of syndrome of phlegm and blood binding.Methods A total of 84 patients with lung metasta-sis of colorectal cancer of syndrome of phlegm and blood binding treated in the Oncology Department of Yantai Hospital of Traditional Chinese Medicine were selected and divided into treatment group and control group according to the random number table,with 42 cases in each group.The control group was treated with mFOLFOX6 chemotherapy regimen,and the treatment group was treated with Fanbei Sanjie granule on the basis of the control group,14 days as one treatment cycles,and both groups were treated for 8 cycles.and the clinical efficacy of the two groups was observed and compared.The tumor control efficacy,scores of traditional Chinese medical syndromes,quality of life[assessed by EORTC QLQ-C30(V3.0)and KPS scores],immune function(CD4+,CD8+,CD4+/CD8+)before and after treatment,and the occur-rence of adverse reactions of chemotherapy of the patients were compared between the two groups.Results After 8 treatment cycles,there was no statistical significance in the objective response rate and disease control rate of patients between the two groups(both P＞0.05);the scores of traditional Chinese medical syndromes,and the scores of fatigue,pain,short-ness of breath,loss of appetite,and diarrhea in the EORTC QLQ-C30(V3.0)scale in the treatment group were signifi-cantly lower than those before treatment and in the control groups(all P＜0.05),and the scores of physical function,role function,emotional function,and general health status in the EORTC QLQ-C30(V3.0)scale were significantly higher than those before treatment and in the control groups(all P＜0.05);the total effective rate of improvement of KPS score in the treatment group was significantly higher than that in the control group[82.05％(32/39)VS.55.26％(21/38),P＜0.05],and CD4+,CD4+/CD8+were significantly higher than that before treatment and in the control group(P＜0.05),while the degree of leukocyte decrease and the occurrence of diarrhea were significantly lower than those in the control group(all P＜0.05).Conclusion Fanbei Sanjie granules combined with mFOLFOX6 chemotherapy regimen can obviously im-prove clinical symptoms,immune function and quality of life,and reduce adverse reactions of patients with lung metastasis of colorectal cancer of syndrome of phlegm and blood binding.
