Efficacy of modified blood-letting puncture and medicine cupping combined with Ba Duan Jin on lumbar dorsal myofasciitis of type of qi stagnation and blood stasis in perimenopausal women
Objective It is to explore the efficacy of modified blood-letting puncture and medicine cupping combined with Ba Duan Jin on lumbar dorsal myofasciitis of type of qi stagnation and blood stasis in perimenopausal women. Methods A total of 122 perimenopausal women with lumbar dorsal myofasciitis of type of qi stagnation and blood stasis treated in Xi-aogan Hospital of Wuhan University of Science and Technology from March 2021 to July 2023 were selected and randomly divided into a control group of 61 cases(6 cases dropped off)and an observation group of 61 cases(4 cases dropped). The control group was treated with modified blood-letting puncture and medicine cupping, the blood-letting puncture was applied at Mingmen(GV4),Yaoyangguan(GV3),bilateral Dachangshu(BL25),bilateral Weizhong(BL40)and Ashi acupoints, after puncturing the collaterals, cupping therapy was performed with medicine cupping made from the decoction for removing blood stasis and relaxing tendons. The observation group was treated with Ba Duan Jin on the basis of the treat-ment in the control group. The treatment frequency of modified blood-letting puncture and medicine cupping in the two groups was once every 3 days, totally treated for 6 times. The Ba Duan Jin training program was conducted in the form of home training combined with regular remote group training, the patients trained at home for 1 hour every day, continuously training for 21 days(participated in remote group training for 1 hour every Saturday). The pain visual analog scale(VAS)score,self-rating anxiety scale(SAS)score, functional disability index(ODI)score before and after 21 days of treatment, and after 3 months of follow-up,and the modified Kupperman score,abdominal/dorsal muscle isometric endurance, and serum levels of interleukin-1β(IL-1 β)and interleukin-6(IL-6)before and after treatment were compared between the two groups, and the clinical efficacy after 21 days of treatment was evaluated and the return rate of 3-month follow-up were cal-culated in the two groups. Results After 21-day treatment and 3-month follow-up,the VAS score,ODI score, and SAS score of the two groups were significantly lower than those before treatment(all P<0.05),and the above scores of the ob-servation group were significantly lower than those of the control group(all P<0.05).After 21-day treatment, the modi-fied Kupperman score and serum levels of IL-1 β and IL-6 of the two groups were significantly lower than those before treat-ment(all P<0.05),and the modified Kupperman score and serum levels of IL-1 β and IL-6 of the observation group were significantly lower than those of the control group(all P<0.05);the isometric endurance of the lumbar/dorsal muscles of both groups were improved significantly compared with before treatment(all P<0.05),and the improvements of the obser-vation group were significantly better than those of the control group(all P<0.05).The total effective rate of the observa-tion group was significantly higher than that of the control group after 21 days of treatment[92.98%(53/57)VS.83.63%(46/55),P<0.05];the return rate of 3-month follow-up of the observation group was significantly lower than that of the control group[17.54%(10/57)VS.34.5%(19/55),P<0.05].Conclusion Modified blood-letting punc-ture and medicine cupping combined with Ba Duan Jin can effectively relieve the low back pain of perimenopausal women with lumbar dorsal myofasciitis of type of Qi stagnation and blood stasis, restore the function of low back, improve the state of perimenopausal syndrome, and reduce the short-term recurrence rate.
