基于网络药理学及实验探讨九味健步饮治疗膝骨关节炎的作用机制

目的 基于网络药理学和动物实验预测及验证九味健步饮治疗膝骨关节炎的潜在作用机制.方法 ①运用数据库筛选九味健步饮每味中药的活性成分和膝骨关节炎的靶点,对九味健步饮与膝骨关节炎的交集靶点进行GO功能富集分析和KEGG通路富集分析,并进行分子对接,挖掘九味健步饮治疗膝骨关节炎的潜在靶点及机制.②将 40 只雄性SD大鼠随机分为空白组,模型组及九味健步饮低、中、高剂量组,每组8 只.除空白组外,其余组大鼠均采用碘乙酸钠关节腔注射方法构建膝骨关节炎模型.造模 2 周后,九味健步饮低、中、高剂量组分别给予 7.78g/(kg·d)、15.56 g/(kg·d)、31.12 g/(kg·d)九味健步饮灌胃,空白组、模型组给予等体积生理盐水灌胃,均连续灌胃2 周.HE染色观察膝关节软骨组织病理形态,ELISA法检测血清中白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平,Western blot法检测软骨组织中磷酸化磷脂酰肌醇3-激酶(p-PI3K)、磷酸化蛋白激酶B(p-Akt)、磷酸化雷帕霉素靶蛋白(p-mTOR)表达情况,验证网络药理学结果.结果 ①九味健步饮有效成分123 个,与疾病作用的共同靶点为248 个,涉及到的通路为细胞凋亡通路、PI3K/Akt信号通路等;分子对接显示核心靶点与其相对应的活性成分结合均具有良好稳定性.②模型组大鼠膝关节软骨表面分层不清,软骨细胞数减少;血清IL-1β、IL-6、IL-8、TNF-α水平和软骨组织中p-PI3K、p-Akt、p-mTOR蛋白相对表达量均明显高于空白组(P均<0.05).九味健步饮各组大鼠膝关节软骨表面分层较清晰,软骨细胞数量较模型组多;血清IL-1β、IL-6、IL-8、TNF-α水平和软骨组织中p-PI3K、p-Akt、p-mTOR蛋白相对表达量均明显低于模型组(P均<0.05),且上述各指标均随着九味健步饮剂量增高而逐渐降低,两两比较差异均有统计学意义(P均<0.05).结论 九味健步饮可能通过调控PI3K/Akt信号通路、细胞凋亡通路等调节IL6、TNF、IL1B、AKT1等靶点的表达,以减轻关节内炎症反应,延缓关节软骨的退变.
Mechanism of action of Jiuwei Jianbu Decoction in the treatment of knee osteoarthritis based on network pharmacology and experiments
Objective It is to predict and verify the potential mechanism of action of Jiuwei Jianbu Decoction in the treatment of knee osteoarthritis based on network pharmacology and animal experiments.Methods①The database was used to screen the active chemical components of each herb of Jiuwei Jianbu Decoction and the targets of knee osteoarthritis,and GO and KEGG enrichment analysis and molecular docking were carried out for the targets,so as to initially explore the po-tential targets and mechanisms of Jiuwei Jianbu Decoction in the treatment of knee osteoarthritis.②Forty male SD rats were randomly divided into blank group,model group and low,medium and high dose group of Jiuwei Jianbu Decoction,with 8 rats in each group.The rats of all groups except for the blank group were injected with sodium iodoacetate into the joint cav-ity to establish models of knee osteoarthritis.After 2 weeks of modeling,the low,medium and high dose group of Jiuwei Jianbu Decoction were given 7.78 g/(kg·d),15.56 g/(kg·d),and 31.12 g/(kg·d)of Jiuwei Jianbu Decoction by gavage,respectively,and the blank and model groups were given equal volumes of saline by gavage,all continuously trea-ted for 2 weeks.The histopathological morphology of knee joint cartilage was observed by HE staining,the serum levels of interleukin-1β(IL-1β),interleukin-6(IL-6),interleukin-8(IL-8),and tumor necrosis factor-α(TNF-α)were detected by ELISA,and the expressions of phosphorylated phosphatidylinositol 3-kinase(p-PI3K),phosphorylated protein kinase B(p-Akt),and phosphorylated rapamycin target protein(p-mTOR)in cartilage tissues were detected by Western blot to ver-ify the network pharmacology results.Results ①There were 123 active ingredients in Jiuwei Jianbu Decoction,with 248 common targets,including apoptosis pathway,PI3K/Akt signaling pathway,etc;the molecular docking showed that the core targets and their corresponding active ingredients had good stability.②The layering of the cartilage surface of knee joint was not clear with reduced number of chondrocytes in the rats of the model group;the levels of serum IL-1β,IL-6,IL-8,TNF-α and the relative expressions of p-PI3K,p-Akt and p-mTOR proteins in the cartilage tissues were significantly higher than those in the blank group(all P<0.05).The layering of the cartilage surface of knee joint was clearer in the rats of each group of Jiuwei Jianbu decoction,and the number of chondrocytes was higher than that of the model group;the levels of serum IL-1β,IL-6,IL-8,TNF-α and the relative expressions of p-PI3K,p-Akt and p-mTOR proteins in the carti-lage tissues were significantly lower than those of the model group(all P<0.05),and the above indexes were decreased more significantly with the increase of the dosage of Jiuwei Jianbu decoction(all P<0.05).Conclusion Jiuwei Jianbu De-coction may regulate the expressions of IL6,TNF,IL1B,AKT1 and other targets through regulating PI3K/Akt signaling pathway and apoptosis pathway,in order to alleviate the inflammatory response in the joints and slow down the degeneration of articular cartilage.
