摘要
目的 观察拖线疗法联合金黄膏治疗低位泛发性肛周脓肿的临床疗效.方法 以2022 年12 月—2023 年10 月上海中医药大学附属龙华医院肛肠科收治的86 例低位泛发性肛周脓肿患者为研究对象,随机分为治疗组与对照组,每组43 例.治疗组采用拖线疗法联合金黄膏进行治疗,对照组单纯采用拖线疗法进行治疗.比较2 组患者术后1 个月的临床疗效、创面愈合时间、余肿消退时间、创面肉芽形态评分、创面渗液情况评分、疼痛VAS评分、Wexner肛门失禁评分及不良反应情况.结果 2 组患者临床疗效比较差异无统计学意义(P>0.05);治疗组创面愈合时间及余肿消退时间均明显短于对照组(P均<0.05);治疗组术后第7 天、第10 天创面肉芽形态与创面渗液情况评分1 分和2 分患者占比均明显高于对照组(P均<0.05);治疗组术后第7 天、第10 天、第14 天创面疼痛VAS评分均明显低于同期对照组(P均<0.05);2 组患者痊愈后3 个月成肛瘘率及成复杂性肛瘘率比较差异均无统计学意义(P均>0.05);2 组患者治疗前后Wexner肛门失禁评分比较差异均无统计学意义(P均>0.05).结论 拖线疗法联合金黄膏治疗泛发性肛周脓肿具有促进创面愈合,加速余肿消退,保护肛门功能等优势.
Abstract
Objective It is to observe the clinical efficacy of thread-drawing therapy combined with Jinhuang ointment in the treatment of low generalized perianal abscess.Methods Eighty-six patients with low generalized perianal abscess treated in the Anorectum Department of Longhua Hospital Affiliated to Shanghai University of Chinese Medicine from De-cember 2022 to October 2023 were randomly divided into treatment group and control group,with 43 patients in each group.The treatment group was treated with thread-drawing therapy combined with Jinhuang ointment,and the control group was treated with thread-drawing therapy alone.The clinical efficacy,wound healing time,residual swelling disappea-ring time,score of wound granulation morphology,score of wound exudate,pain VAS score,Wexner fecal incontinence score and adverse reactions of the patients in the two groups were compared at 1 month after surgery.Results There was no statistically significant difference in the clinical efficacy between the two groups(P>0.05);the wound healing time and residual swelling disappearing time of the treatment group were significantly shorter than those of the control group(both P<0.05);the percentage of patients with scores of 1 and 2 for granulation morphology and wound exudate of the treatment group were significantly higher than those of the control group on the 7th and 10th postoperative days(P<0.05);the VAS scores of wound pain on the 7th,10th,and 14th postoperative days of the treatment group were significantly lower than those of the control group during the same period(all P<0.05);there were no significant differences in the fistula rate and recurrent fistula rate between the two groups in 3 moths after recovery(both P>0.05);there was no significant differ-ence in the Wexner fecal incontinence score before and after the treatment between the two groups(P>0.05).Conclusion Thread-drawing therapy combined with Jinhuang ointment has the advantages of promoting wound healing,accelerating the regression of residual swelling,and protecting anal function in the treatment of generalized perianal abscess.
