摘要
目的:探讨60岁以上退行性心脏瓣膜病(degenerative valvular disease,DHVD)患者血清胱抑素-C(cystatin C,Cys-C)、Hcy、胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)的表达特征,并分析其与瓣膜钙化的关系.方法:选取我院接诊的306例60岁以上DHVD患者作为研究对象,根据超声心动图检查结果分为瓣膜钙化组(237例)与非瓣膜钙化组(69例).所有患者均测定血清Cys-C、Hcy、IGF-1水平,对比两组的检查结果,并分析影响瓣膜钙化的危险因素.结果:瓣膜钙化组与非瓣膜钙化组在性别构成、饮酒率、吸烟率、合并高血压、合并糖尿病、FBG、HDL-C水平比较,差异均无统计学意义(P>0.05).与非瓣膜钙化组相比,瓣膜钙化组的年龄、TC、TG、LDL-C水平更高(P<0.05).瓣膜钙化组的血清Cys-C、Hcy、IGF-1水平均显著高于非瓣膜钙化组(P<0.05).多因素回归分析显示,年龄、LDL-C、Cys-C、Hcy和IGF-1均是瓣膜钙化的危险因素(P<0.05).结论:60岁以上DHVD患者血清Cys-C、Hcy、IGF-1表达的升高与瓣膜钙化密切相关,监测血清Cys-C、Hcy、IGF-1浓度可为临床预测DHVD瓣膜功能和制定治疗方案提供参考依据.
Abstract
Objective:To investigate the expression characteristics of serum cystatin C(Cys C),Hcy and insulin-like growth factor-1(IGF-1)in over 60s patients with degenerative valvular disease(DHVD),and analyze the relationship between them and valve calcification.Methods:A total of 306 cases of over 60s patients with DHVD admitted in our hospital were selected as the study subjects.According to the results of echocardiography,they were divided into valvular calcification group(79 cases)and non valvular calcification group(23 cases).Serum Cys-C,Hcy,IGF-1 levels were measured in all patients.The results of the two groups were compared,and the risk factors affecting valve calcification were analyzed.Results:There was no significant difference between the valvular calcification group and the non valvular calcification group in gender composition,drinking rate,smoking rate,hypertension,diabetes,FBG,HDL-C levels(P>0.05).Compared with the non valvular calcification group,the age,TC,TG and LDL-C levels in the valvular calcification group were higher(P<0.05).Serum Cys-C,Hcy and IGF-1 levels in patients with valvular calcification were significantly higher than those in patients without valvular calcification(P<0.05).Multivariate regression analysis showed that age,LDL-C,Cys-C,Hcy and IGF-1 were all risk factors for valve calcification(P<0.05).Conclusions:The increase of serum Cys-C,Hcy,IGF-1 expression in over 60s patients with DHVD is closely related to valve calcification.Monitoring serum Cys-C,Hcy,IGF-1 concentration can provide a reference for clinical prediction of valve function and formulation of treatment plan for DHVD.
