环状核糖核酸转化/转录域关联蛋白和微小核糖核酸-761在冠心病中的表达及临床意义
Expression and clinical significance of circular RNA circ transcription domain associated protein and microrNA-761 in coronary heart disease

摘要
目的:探究环状核糖核酸转化/转录域关联蛋白(circular RNA transformation/transcription domain associated protein,circTRRAP)和微小 RNA-761(microRNA-761,miR-761)在冠心病患者血清中的表达及与心肌损伤的相关性.方法:从2019年12月到2022年12月,在湖南中医药大学第一附属医院接受治疗的80例冠状动脉疾病患者被纳入研究组,同时,80名健康人为对照组.实时荧光定量PCR法用来检测血清中circTRRAP、miR-761表达水平;检测两组心肌损伤相关指标cTnI、cTnT、CK-MB和BNP的水平;采用Person相关性系数分析circTRRAP、miR-761表达与心肌损伤指标的相关性;以ROC曲线探究血清circTRRAP和miR-761水平对冠心病的诊断价值.结果:观察组的circTRRAP水平显著高于对照组,而miR-761水平则远低于对照组(P<0.05).在心肌损伤指标cTnI、cTnT、CK-MB以及BNP方面,观察组的表达较对照组明显升高(P<0.05).Pearson法结果显示,冠心病患者血清circTRRAP水平与miR-761呈负相关(r=-0.422,P<0.05).此外,冠心病患者血清circTRRAP表达水平与cTnI、cTnT、CK-MB和BNP呈显著性的正相关(P<0.05);miR-761 表达水平与cTnI、cTnT、CK-MB和BNP呈显著性的负相关(P<0.05).circTRRAP水平诊断冠心病的曲线下面积(AUC)为0.917,敏感度为78.3%,特异性为95.0%;miR-761水平诊断冠心病的AUC为0.907,敏感度为97.8%,特异性为72.5%;circTRRAP和miR-761联合诊断的AUC为0.991,敏感度为93.5%,特异性为97.5%.结论:冠心病患者血清中circTRRAP的表达水平较高,miR-761的表达水平较低,且血清circTRRAP和miR-761表达水平对冠心病患者早期诊断均有重要作用.
Abstract
Objective:To investigate the expression of circular RNA transformation/transcription domain associated protein(circTRRAP)and microRNA-761(miR-761)in serum of patients with coronary heart disease(CHD)and their correlation with myocardial injury.Methods:From December 2019 to December 2022,80 patients with coronary artery disease treated in our hospital were included in the study group,and at the same time,80 healthy individuals were selected as the control group.Real-time quantitative PCR was used to detect the expression levels of circTRRAP and miR-761 in serum.The levels of myocardial injury-related indexes cTnI,cTnT,CK-MB and BNP in the two groups were detected.Person correlation coefficient was used to analyze the correlation between circTRRAP,miR-761 expression and myocardial injury indicators.ROC curve was used to explore the diagnostic value of serum circTRRAP and miR-761 levels for coronary heart disease.Results:The level of circTRRAP in the observation group was significantly higher than that in the control group,while the level of miR-761 was much lower than that in the control group(P<0.05).The expressions of myocardial injury indexes cTnI,cTnT,CK-MB and BNP in the observation group were significantly higher than those in the control group(P<0.05).Pearson analysis showed that serum circTRRAP expression level was negatively correlated with miR-761 in patients with CHD(r=-0.422,P<0.05).In addition,serum circTRRAP levels in patients with coronary heart disease were positively correlated with cTnI,cTnT,CK-MB and BNP(P<0.05).The expression level of miR-761 was negatively correlated with cTnI,cTnT,CK-MB and BNP(P<0.05).The area under the curve(AUC)of circTRRAP level in diagnosing CHD was 0.917,the sensitivity was 78.3%,and the specificity was 95.0%.The AUC of miR-761 level in diagnosing CHD was 0.907,the sensitivity was 97.8%,and the specificity was 72.5%.The AUC of combined diagnosis of circTRRAP and miR-761 was 0.991,the sensitivity was 93.5%,and the specificity was 97.5%.Conclusions:The expression level of serum circTRRAP is higher and the expression level of miR-761 is lower in patients with CHD,and the expression levels of serum circTRRAP and miR-761 play an important role in the early diagnosis of CHD patients.
