Predictive value of red blood cell distribution width and growth differentiation factor-15 in poor prognosis of patients with unstable angina
Objective To investigate the predictive value of red blood cell distribution width(RDW)and growth differentiation factor-15(GDF-15)in the poor prognosis of patients with unstable angina(UA).Methods A total of 436 UA patients admitted to the department of cardiovascular medicine of Fourth People's Hospital of Langfang from March 2020 to October 2022 were selected,and divided into the good prognosis group(358 cases)and the poor prognosis group(78 cases)according to whether adverse cardiovascular events occurred during the follow-up period.Serum expression levels of RDW and GDF-15 and other clinical features were compared between the two groups.Multivariate Logistic regression analysis was used to analyze the risk factors for poor prognosis of UA patients.Receiver operating characteristic(ROC)curve analysis was used to analyze the value of RDW and GDF-15 for predicting poor prognosis in UA patients.Results There were statistically significant differences in total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),serum creatinine(Scr),C-reactive protein(CRP),RDW,GDF-15,left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),and the proportion of chronic occlusive lesions between the two groups(t/χ2=2.078,2.566,5.329,7.161,6.819,15.928,7.817,4.489,5.662;P<0.05).Multivariate Logistic regression analysis showed that RDW,GDF-15,and LVEDD were risk factors for poor prognosis in UA patients(OR=2.911,2.287,1.263;P<0.05).The area under curve(AUC)of RDW,GDF-15 and their combination in predicting poor prognosis of UA patients were 0.695,0.713 and 0.817,respectively.The value of combined prediction was higher than that of the individual indicators(Z=2.691,2.603;P<0.01).Conclusion Increased expression levels of RDW and GDF-15 are risk factors for poor prognosis in UA patients,and both have predictive value for poor prognosis,with better efficacy in combined prediction.
