Clinical value of combined detection of serum salusin-β,CX3CL1 and soluble growth stimulation expressed gene 2 protein in the diagnosis and prognosis evaluation of acute heart failure
Objective To investigate the expression of salusin-β,CX3CL1,and soluble growth stimulation expressed gene 2 protein(sST2)in the serum of patients with acute heart failure(AHF),and to assess the clinical value of their combination in the diagnosis and prognostic assessment of AHF.Methods A total of 225 patients with AHF admitted to Cangzhou People's Hospital from December 2020 to December 2021 were selected as the observation group,and 120 healthy individuals undergoing physical examination were selected as the control group.The serum levels of salusin-β,CX3CL1 and sST2 were measured by enzyme-linked immunosorbent assay(ELISA).The correlation between serum levels of salusin-β,CX3CL1,and sST2 and left ventricular ejection fraction(LVEF)in patients with AHF was analyzed using Pearson correlation analysis.The clinical value of salusin-β,CX3CL1,and sST2 in the diagnosis and prognosis evaluation of AHF was analyzed using ROC curve analysis,and the relationship between salusin-β,CX3CL1,sST2,and AHF survival rates was analyzed using Kaplan-Meier curve.Results Serum levels of salusin-β,CX3CL1,and sST2 in the observation group were higher than those in the control group,and LVEF was lower than that in the control group(t=7.788,6.960,8.610,13.908;P<0.05).Serum levels of salusin-β,CX3CL1,and sST2 decreased sequentially in the heart failure with reduced ejection fraction(HFrEF)group,heart failure with mid-range ejection fraction(HFmrEF)group,and heart failure with preserved ejection fraction(HFpEF)group,with statistically significant differences among groups(F=32.725,31.224,36.308;P<0.05).Pearson correlation analysis showed that serum levels of salusin-β,CX3CL1,and sST2 were negatively correlated with LVEF in AHF patients(r=-0.364,-0.524,-0.382;P<0.05).Levels of salusin-β,CX3CL1,and sST2 were higher in the poor prognosis group than those in the good prognosis group,with statistically significant differences(t=9.137,8.242,7.155;P<0.05).The ROC curve showed that the combined diagnosis of HFrEF in AHF patients using salusin-β,CX3CL1,and sST2 had an AUC of 0.848.The combined evaluation of prognosis using salusin-β,CX3CL1,and sST2 had an AUC of 0.935,with sensitivity and specificity of 92.90%and 80.00%,respectively.The one-year survival rates in the high-expression groups of salusin-β,CX3CL1,and sST2 were lower than those in the low-expression groups(Log rank=68.342,77.547,70.883;P<0.05).Conclusion The expressions of salusin-β,CX3CL1 and sST2 in the serum of patients with AHF are up-regulated.The combination of these markers can better diagnose AHF and evaluate prognosis,and can be used as biological markers for early diagnosis and prognosis evaluation of AHF.
