Diagnostic value of echocardiography combined with serum DNA methyltransferase 1 and long non-coding human maternal expression gene 3 in chronic heart failure
Objective To investigate the diagnostic value of three-dimensional echocardiography combined with serum DNA methyltransferase 1(DNMT1)and long non-coding human maternal expression gene 3(LncRNA MEG3)in chronic heart failure(CHF).Methods Ninety patients with CHF admitted to the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture from December 2018 to December 2022 were selected as the observation subjects(CHF group).According to the New York Heart Association(NYHA)cardiac function classification,there were 32 cases of gradeⅡ,36 cases of gradeⅢ,and 22 cases of gradeⅣ.In addition,90 healthy individuals undergoing physical examination during the same period were selected as the control group.Serum DNMT1 level was detected by enzyme-linked immunosorbent assay(ELISA),and serum LncRNA MEG3 expression was detected by real-time quantitative PCR(RT-qPCR).Correlations between serum DNMT1,LncRNA MEG3 levels,and various indexes in CHF patients were analyzed by Pearson correlation analysis,and the diagnostic value of echocardiography indexes combined with serum DNMT1 and LncRNA MEG3 levels for CHF was analyzed by receiver operating characteristic(ROC)curve.Results Compared with the control group,the levels of left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV),the standard deviation of the time to minimum systolic volume at 16 segments of the left ventricle(Tmsv16-SD),the maximum time difference of the time to minimum systolic volume at 16 segments of the left ventricle(Tmsv16-Dif),serum DNMT1,and LncRNA MEG3 in the CHF group significantly increased(t=9.326,18.904,5.637,8.326,10.794,13.140;P<0.01),while left ventricular ejection fraction(LVEF)significantly decreased(t=13.118,P<0.01).Compared with gradeⅡ patients,the levels of LVEDV,LVESV,Tmsv16-Dif,Tmsv16-SD,serum DNMT1,and LncRNA MEG3 in gradeⅢand grade Ⅳ patients significantly increased(P<0.05),while LVEF level significantly decreased(P<0.05).Compared with gradeⅢ patients,the levels of LVEDV,LVESV,Tmsv16-Dif,Tmsv16-SD,serum DNMT1,and LncRNA MEG3 in gradeⅣ patients significantly increased(P<0.05),while LVEF level significantly decreased(P<0.05).Serum DNMT1 and LncRNA MEG3 levels in CHF patients were positively correlated with LVEDV,LVESV,Tmsv16-Dif,and Tmsv16-SD(r=0.581,0.475,0.563,0.628,0.617,0.557,0.593,0.603;P<0.01)and negatively correlated with LVEF(r=-0.521,-0.498;P<0.01).The AUC for the combined diagnosis of CHF using echocardiographic indexes,serum DNMT1,and LncRNA MEG3 was 0.984(95%CI=0.953-0.997),with a sensitivity of 95.56%and specificity of 93.33%,which was higher than that of DNMT1 or LncRNA MEG3 alone(Z=5.813,5.098;P<0.05).Conclusion The combined detection of echocardiographic indexes,DNMT1,and LncRNA MEG3 has significant diagnostic value for CHF.
