Early Clinical Value of Comparative Anterolateral Approach Hemiarthro-plasty for Treatment of Fractures of Femoral Neck in Advanced Ages
Objective To analyze the effect of implementing anterolateral approach hemiarthroplasty treatment for femoral neck fracture in advanced age.Methods A total of 86 elderly patients with femoral neck fracture admitted to Wenshang County People's Hospital from October 2021 to October 2023 were selected as the study objects,all of whom underwent semi-hip replacement.According to different approaches,they were divided into anterior and poste-rior groups with 43 cases in each group.The anterolateral approach was taken in the anterior group,and the posterolat-eral approach was taken in the posterior group.Bone metabolism,clinical indexes,hip function and complications were compared between the two groups.Results After operation,osteocalcin,carboxyl terminal propeptide of type Ⅰ procollagen and amino terminal propeptide of type Ⅰ procollagen were increased in both groups,and the differences were statistically significant(all P<0.05).After surgery,osteocalcin[(30.02±2.97)ng/mL],carboxy-terminal propep-tide of type Ⅰ procollagen[(27.74±3.52)ng/mL],amino terminal propeptide of type Ⅰ procollagen[(27.45±2.35)ng/mL]were higher in the anterior group than in the posterior group,and the differences were statistically significant(t= 11.645,8.028,11.093,all P<0.05).The operation time of the anterior group was longer than that of the posterior group,the blood loss and drainage volume of the anterior group were lower than those of the posterior group,and the incision length,exit time and discharge time of the anterior group were shorter than those of the posterior group,and the differences were statistically significant(all P<0.05).There was no significant difference in the rate of hip function and complication between the two groups(both P>0.05).Conclusion Both approaches can maintain a high rate of hip joint excellence with a low risk of complications,but the anterolateral approach has higher value in terms of recovery cycle and bone metabolism,and is more suitable for elderly patients with femoral neck fractures.
