目的 探讨白内障超声乳化(Phacoemulsification of Cataract,BCI)、人工晶体植入术(Implantable Collamer Lens,ICL)联合小梁切除术治疗原发性闭角型青光眼合并白内障的效果.方法 选取2019年1月—2023年10月南京市六合区人民医院诊治的76例原发性闭角型青光眼合并白内障患者为研究对象,按随机数表法分为对照组和观察组,各38例.对照组行小梁切除术治疗,观察组联合BCI及ICL治疗,对比两组手术治疗效果.结果 观察组治疗总有效率高于对照组(94.74%vs 76.32%),差异有统计学意义(χ2=5.208,P<0.05).与对照组比,观察组术后视力更优,前房深度、前房角度更大,眼压更低,差异有统计学意义(P均<0.05).两组并发症发生率对比,差异无统计学意义(P>0.05).结论 原发性闭角型青光眼合并白内障采取小梁切除术联合BCI+ICL的疗效可靠,安全性较高.
Analysis of Clinical Effect of Phacoemulsification of Cataract+Implantable Collamer Lens Implantation Combined with Trabeculectomy for Primary Angle-closure Glaucoma Combined with Cataract
Objective To investigate the effect of phacoemulsification of cataract(BCI),implantable collamer lens im-plantation(ICL)combined with trabeculectomy in the treatment of primary angle-closure glaucoma combined with cataract.Methods 76 patients of primary angle-closure glaucoma combined with cataract diagnosed and treated in Nanjing Liuhe District People's Hospital from January 2019 to October 2023 were selected as the study objects and were divided into control group and observation group according to the random number table method,with 38 cases in each group.The contol group was treated with trabeculectomy,and the observation group combined BCI+ICL.The sur-gical treatment effects of the two groups were compared.Results The total effective rate in the observation group was higher than that in the control group(94.74%vs 76.32%),the difference was statistically significant(χ2=5.208,P<0.05).Compared with the control group,the observation group had better postoperative visual acuity,higher anterior chamber depth and anterior chamber angle,while the intraocular pressure was lower,the differences were statistically significant(all P<0.05).There was no statistically significant difference in the complication rate between the two groups(P>0.05).Conclusion Primary angle-closure glaucoma combined with cataract by trabeculectomy,BCI+ICL has reliable efficacy and high safety.
