子宫肌壁大部切除联合曼月乐环治疗子宫腺肌病的临床疗效探讨
Clinical Effect of Subtotal Myowall Resection Combined with Mirena Ring in the Treatment of Adenomyosis

摘要
目的 分析子宫肌壁大部切除联合曼月乐环治疗子宫腺肌病的临床疗效.方法 回顾性选取2018年10月—2023年10月遵义市妇幼保健院收治的80例子宫腺肌病患者的临床资料,按治疗方式不同分为对照组、观察组,各40例.所有患者均行经腹子宫肌壁大部切除及子宫重建术,观察组联合曼月乐环治疗.比较两组疗效、月经失血图(Pictorial Blood Loss Assessment Chart,PBAC)评分及复发率.结果 观察组治疗总有效率(95.00%)高于对照组(80.00%),差异有统计学意义(χ2=4.114,P<0.05).治疗前两组PBAC评分比较,差异无统计学意义(P>0.05);术后60、90、120 d,观察组PBAC评分低于对照组,差异有统计学意义(P均<0.05).观察组复发率(2.50%)低于对照组(15.00%),差异有统计学意义(χ2=3.914,P<0.05).结论 子宫肌壁大部切除联合曼月乐环与单独手术治疗子宫腺肌病相比具有更好的改善效果,有效减少月经量,获得较高的治疗有效率,且术后复发率较低.
Abstract
Objective To analyze the clinical effect of subtotal myowall resection combined with mirena ring in the treatment of adenomyosis.Methods Clinical data of 80 patients with adenomyosis admitted to Zunyi Maternal and Child Health Hospital from October 2018 to October 2023 were retrospectively selected,and divided into control group and observation group according to different treatment methods,with 40 cases in each group.All patients under-went transabdominal myomectomy and uterine reconstruction surgery,and the observation group was treated with a combination of mirena ring.The therapeutic effects,Menstrual Blood Loss Assessment Chart(PBAC)scores,and recur-rence rates between two groups were compared.Results The total effective rate of the observation group(95.00%)was higher than that of the control group(80.00%),and the difference was statistically significant(χ2=4.114,P<0.05).There was no statistically significant difference in PBAC scores between the two groups before treatment(P>0.05).At 60,90,and 120 days after surgery,the PBAC scores of the observation group were lower than those of the control group,and the differences were statistically significant(all P<0.05).The recurrence rate of the observation group(2.50%)was lower than that of the control group(15.00%),and the difference was statistically significant(χ2=3.914,P<0.05).Conclusion Compared with the single operation in the treatment of adenomyosis,myowall resection combined with mirena ring has a better improvement effect,effectively reduces menstrual volume,obtains a higher treatment effi-ciency,and has a lower postoperative recurrence rate.
