目的 探讨血必净联合血液净化治疗在重症急性胰腺炎(Severe Acute Pancreatitis,SAP)合并多器官功能障碍综合征(Multiple Organ Dysfunction Syndrome,MODS)患者中的应用价值.方法 回顾性选取2020年10月—2023年10月南通市中医院收治的80例SAP合并MODS患者的临床资料,根据治疗方法的差异性进行分组,每组40例,对照组在常规治疗的基础上加用血液净化治疗,观察组在对照组的基础上加用血必净.比较两组临床症状缓解情况、MODS评分、急性胰腺炎床边严重度指数(Bedside Index for Severity in Acute Pancre-atitis,BISAP)、C-反应蛋白(C-reactive Protein,CRP)、白细胞介素-6(Interleukin-6,IL-6)、氧合指数与血清肌酐水平.结果 观察组BISAP评分、MODS评分为(1.44±0.47)分、(2.95±0.95)分,低于对照组的(2.06±0.54)分、(3.97±0.97)分,差异有统计学意义(t=5.477、4.751,P均＜0.05).观察组的临床症状缓解时间均短于对照组,氧合指数高于对照组,CRP、IL-6与肌酐水平低于对照组,差异有统计学意义(P均＜0.05).结论 在SAP合并MODS患者治疗中,采用血必净联合血液净化治疗,能够缩短主要症状的缓解时间,改善患者的病情,减轻炎症反应,对呼吸、肾脏损伤也有一定的保护作用.
Analysis of the Effect of Xuebijing Combined with Blood Purification in the Treatment of Severe Acute Pancreatitis Combined with MODS
Objective To explore the application value of Xuebijing combined with blood purification treatment in pa-tients with severe acute pancreatitis(SAP)combined with multiple organ dysfunction syndrome(MODS).Methods The clinical data of 80 patients with SAP combined with MODS admitted to Nantong Hospital of Traditional Chinese Medicine from October 2020 to October 2023 were retrospectively selected and grouped according to the differences in treatment methods,with 40 cases in each group.The control group was treated with blood purification on the basis of conventional treatment,and the observation group was treated with Xuebijing on the basis of the control group.The two groups were compared in terms of clinical symptom relief,MODS score,bedside index for severity in acute pancre-atitis(BISAP),C-reactive protein(CRP),interleukin-6(IL-6),oxygenation index and serum creatinine level.Results The scores of BISAP score and MODS score of the observation group were 1.44±0.47 and 2.95±0.95 points,which were lower than 2.06±0.54 and 3.97±0.97 of the control group points and points,and the differences were statistically significant(t=5.477,4.751,both P＜0.05).In the observation group,the time of clinical symptom relief was shorter than that of the control group,the oxygenation index was higher than that of the control group,and the levels of CRP,IL-6 and creatinine were lower than that of the control group,and the differences were statistically significant(all P＜0.05).Conclusion In the treatment of patients with SAP combined with MODS,the use of Xuebijing combined with blood purification treatment can shorten the time of relief of the main symptoms,improve the patient's condition,re-duce the inflammatory response,and also have a certain protective effect on respiratory and renal damage.
