目的 探讨系统免疫炎症指数(systemic immune inflammation index,SII)和中性粒细胞与淋巴细胞比值(neutrophi to lymphocyte ratio,NLR)对急性缺血性卒中相关性肺炎(stroke-associated pneumonia,SAP)的预测价值.方法 回顾性选取江苏大学附属武进医院于2020年1月—2022年12月收治的238例急性缺血性脑卒中患者的临床资料,其中118例发生急性缺血性SAP的患者为SAP组,未发生SAP的急性缺血性脑卒中患者120例作为非SAP组.利用多因素Logistic回归分析找出急性缺血性SAP的危险因素.绘制受试者工作特征曲线(receiver operating characteristic curve,ROC),并根据ROC曲线下面积(area under curve,AUC)、敏感度和特异度去分析SII、NLR单一及其联合检测在急性缺血性SAP中的预测价值.结果 多因素分析结果显示,SII和NLR是SAP发生的独立危险因素,差异有统计学意义(OR:1.101、1.287;95%CI:1.042~1.182、1.055~1.568,P均<0.05);ROC分析发现SII、NLR的AUC分别为0.807、0.819,而两者联合检测的AUC为0.825,两者联合检测的特异度、敏感度明显高于单一项目的测定.结论 高NLR和SII是SAP的独立危险因素,SII和NLR联合检测对SAP有更高的预测价值.
Predictive Value of Systemic Immune Inflammation Index and Neutrophil Lymphocyte Ratio for Acute Ischemic Stroke Associated Pneumonia
Objective To investigate the predictive value of systemic immune inflammation index(SII)and neutrophil to lymphocyte ratio(NLR)for acute ischemic stroke-associated pneumonia(SAP).Methods Clinical data of 238 pa-tients with acute ischemic stroke admitted to Wujin Hospital Affiliated to Jiangsu University from January 2020 to De-cember 2022 were retrospectively selected.Among them,118 patients with acute ischemic SAP were SAP group,and 120 patients with acute ischemic stroke without SAP were non-SAP group.Multivariate Logistic regression analysis was used to find out the risk factors of acute ischemic SAP.The receiver operating characteristic curve(ROC)was drawn,and the predictive value of single and combined detection of SII and NLR in acute ischemic SAP was analyzed according to the area under curve(AUC)of the ROC,sensitivity and specificity.Results Multivariate analysis showed that SII and NLR were independent risk factors for SAP,and the difference was statistically significant(OR:1.101,1.287;95% CI:1.042-1.182,1.055-1.568,both P<0.05).ROC curve analysis showed that the AUC of SII and NLR were 0.807 and 0.819,respectively,while the AUC of the combined detection was 0.825.The specificity and sensitiv-ity of the combined detection were significantly higher than those of the single item.Conclusion High NLR and SII are independent risk factors for SAP,and the combined detection of SII and NLR has higher predictive value for SAP.
